About this file

This is a Purchase Continuation Sheet (WV-36) issued by the State of West Virginia for a Request for Quotations (RFQ) related to William R. Sharpe, Jr. Hospital. The solicitation requires vendors to attend a mandatory pre-bid meeting scheduled for 9:00 AM on Tuesday, June 18, 2024, at William R. Sharpe, Jr. Hospital, where attendees should notify Bryan Gustke upon arrival. The document indicates that pricing will not be discussed during the pre-bid meeting, and bids will not be accepted via email. Vendors are encouraged to submit bids through wvOASIS, with technical support available through the helpdesk at 855-666-8823. The document requests standard vendor information including name, address, signature, phone, fax, and email contact details for bid submission purposes. No specific pricing terms, set-asides, incumbent information, funding amounts, or contract duration details are provided in this continuation sheet.
