Attachment #8 - Past Performance Questionnaire Solicitation W91CRB-25-R-0007 for NVD-N
1. The U.S. Army Contracting Command - Aberdeen Proving Ground Acquisition Center (ACC-APG) is currently conducting a source selection for the NVD-N which reflects an acquisition strategy developed by the Product Manager Soldier Maneuver Sensors in coordination with Program Executive Office Soldier and supporting organizations. As part of this source selection process, a Past Performance team has been established to evaluate the past performance of Offerors submitting proposals and their proposed subcontractors. An Offeror's performance record on past and current contracts is a key factor in evaluating the Offeror's likelihood of success on future contracts.
2. To aid this evaluation process, Offerors are required by the RFP to request the POCs listed in Factor 5 of their proposals to complete this Past Performance Questionnaire. The information provided will be used in the awarding of a Government contract; therefore, it is important that the information be as complete, factual and accurate as possible. Upon completion of this questionnaire by the Respondent POC, request the POC send the completed questionnaire via electronic mail to the ACC-APG Contracts at e-mail: michael.c.james78.civ@army.mil and gerald.a.simmons2.civ@army.mil within 30 calendar days of receipt. The questionnaire is NOT to be submitted back to the Offeror, either as an original or as a copy. Request the Respondent POC maintain the original completed/signed questionnaire as a record of its response.
3. In completing this questionnaire, Respondent POCs should be aware that the Past Performance team will conduct the performance assessment for this acquisition based on the quality, relevance, and recentness of the Offeror's past performance, as it relates to the probability of successful accomplishment of the required effort. When assessing performance risk, the evaluation team will focus its inquiry on the past performance of the Offeror and proposed major subcontractors as it relates to all solicitation requirements. These requirements include all aspects of cost, schedule and performance, including the Offeror’s record of: 1) conforming to specifications and standards of good workmanship; 2) adherence to contract schedules, including the administrative aspects of performance; 3) maintaining program execution within contract cost (to include firm fixed price); 4) ability to resolve technical and manufacturing problems quickly and effectively; 5) establishing and maintaining adequate management of subcontractors; 6) business-like concern for the interest of its customers; and 7) meeting program quality objectives.
4. Part I of the questionnaire is to be completed by the Offeror or its proposed subcontractor. Parts II and Ill are to be completed by the applicable Respondent POCs.
