1 INTRODUCTION
On April 29, 2013, Lehua Environmental Consultants, LLC (LEC) personnel performed a limited asbestos survey and asbestos screen at the Subject Location. The purpose of this survey and screen was to investigate the Subject Location for the presence of asbestos-containing materials (ACM) and suspect ACM. Specifically, LEC completed the following tasks:
Performed site reconnaissance at the Subject Location;
Collected twenty-one (21) samples of suspect ACM from selected areas of the
Subject Location;
Submitted the twenty-one (21) samples of suspected ACM to NVL Laboratories, Inc.
for analysis of asbestos via Polarized Light Microscopy (PLM) in accordance with the AHERA protocol and NIOSH Method 600/R-93/116;
Visually inspected selected accessible areas of the Subject Location for the presence of suspect ACM;
Prepared this report documenting the field activities and the results of the investigation including analytical results, visual observations, photographs, conclusions, and recommendations.
This survey is not a comprehensive asbestos survey.
This asbestos survey did not include sampling of all identified suspect ACM. Visual identification and cataloging of suspect ACM not included in the sample collection, is provided in Table 2 of this report. The scope for identifying suspect ACM was limited to five (5) suspect areas (15 samples); however, more than five (5) areas were identified as suspect ACM. At the time of demolition, additional analysis will be required to complete the demolition notification requirements for no asbestos.
2 METHODOLOGY
2.1 Asbestos Bulk Survey
LEC personnel collected a total of twenty-one (21) samples of suspect building materials for asbestos analysis. All of the suspect ACM samples were collected from various areas of the Subject Location in general accordance with EPA guidelines and recommendations.
The suspect asbestos-containing materials were carefully cut out and placed into a labeled resealable plastic bag. The sampling equipment was cleaned between each sample collection to avoid cross-contamination between samples. The approximate quantity of each suspect asbestos-containing material was noted. Sample locations were randomly selected in accordance with EPA protocols and recommendations.
All samples were properly logged and recorded following strict chain of custody procedure and submitted to NVL Laboratories, Inc. in Seattle, Washington for analysis by polarized light microscopy in accordance with EPA Method 600/R-93/116. NVL
Laboratories, Inc. is accredited for bulk asbestos analysis through successful participation in the National Voluntary Lab Accreditation Program (NVLAP).
2.2 Asbestos Screen
LEC personnel visually inspected selected accessible areas at the Subject Location.
Materials suspected to contain asbestos were documented with a description and evaluation of the material’s condition, estimated quantity and homogeneous location.
3 RESULTS
3.1 Asbestos Bulk Survey
A total of twenty-one (21) asbestos samples were collected and submitted for analysis for via PLM. None of the sampled materials contained levels of asbestos above the regulatory limit of 1%. Table 1 below lists the results of all samples collected.

Asbestos Screen
LEC’s visual inspection of the Subject Location identified twenty-six (26) materials which are suspected to contain asbestos. These materials should be considered ACM until further sampling and analysis is completed. Table 2 below summarizes these results.

4 RECOMMENDATIONS
In summary, various materials suspected to contain asbestos were observed in the areas inspected. Based on LEC’s visual inspection of the Subject Location, LEC recommends the following:
Assume all suspect asbestos-containing materials are ACM until sampling and analysis determines otherwise.
Conduct sampling and analysis of suspected asbestos-containing materials prior to renovation or demolition activities that may disturb these materials to determine the actual presence of asbestos.
All friable ACM must be removed and disposed of by a qualified asbestos abatement contractor. Friable ACM is defined as those materials that may be crumbled, pulverized, or otherwise damaged by hand pressure.
Any non-friable ACM, which could be crumbled and pulverized during renovation/demolition activities must be removed and disposed of by a qualified asbestos abatement contractor. Non-friable ACM is pliable (not brittle), breaks by tearing rather than fracturing, and does not easily release asbestos fibers upon breaking. In addition, the services of a qualified consultant should be obtained to monitor and inspect the removal activities to ensure compliance with applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), and Hawaii Occupational Safety and Health (HIOSH) regulations pertaining to the handling of asbestos containing material.
Any abatement and demolition contractor(s) must take appropriate measures to comply with applicable EPA, OSHA and HIOSH regulations pertaining to the handling of asbestos containing materials and worker protection.
Have air monitoring conducted for airborne asbestos fibers by a State of Hawaii certified
Project Monitor during any asbestos abatement and general renovation/demolition activities of areas that were determined to contain this contaminant.
