3. DESCRIPTION/PURPOSE
These reports summarize the contractors weekly expenditures and status of each Task Authorization Request (TAR)

This data item description (DID) is uniquely applicable to OTC’s Test Support Contract (TSC). It provides data required by specific OTC entries.

10. PREPARATION INSTRUCTIONS
10.1 Provide the following information by each Task Authorization Request (TAR) , on a weekly basis:
a. Summary of Events ( happened during that week)
b. Test Concerns or Issues
c. Personnel Issues
d. Next Week’s Events
e. Milestones
f. Financial Report- include TIP costing, Funded, Spent, Available amount, percentage spent to date, additional funding required (yes or no)
g. Accumulative Hours Expended – include labor code, regular, overtime expended compared to TIP costing
h. Staffing- include position, name, email, phone
10.2 These costs will be broken into labor, fee, and reimbursable costs.
10.3 Additional spreadsheets will be provided for any TAR/TASK Implantation Letter (TIL) requests to track specific costs.
10.4 Provide a power point that includes: Staffing per labor category, timelines, position, number of personnel per labor category
10.5 Test Work Schedule that depicts milestones, events, dates, hours for each event

The public reporting burden for this collection of information is estimated to average 110 hours per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information.
