About this file
This is a Past Performance Assessment Questionnaire template used by the U.S. Army Contracting Command - Rock Island (ACC-RI) to evaluate contractor performance on federal contracts. The questionnaire was issued on October 20, 2026, in support of RFP W519TC-26-R-A076 and requests references to provide candid assessments of an offeror's performance on recent and relevant federal, state, or local government and commercial contracts involving services under NAICS 541612 or similar efforts. Respondents must return completed questionnaires to the Contracting Officer (Brooke B. Sheraden) and Contract Specialist (Kynndel M. Wallarab) within fifteen (15) calendar days of receipt.
The questionnaire evaluates contractor performance across six key areas: compliance of products, services, documents, and deliverables to specification requirements and workmanship standards; effectiveness of project management including subcontractor control; timeliness of performance for services and product deliverables; effectiveness in forecasting and controlling estimated costs (for cost-reimbursement contracts only); commitment to customer satisfaction and business-like concern for customer interests; and general comments on other relevant performance information. Respondents rate performance as Acceptable, Unacceptable, or Unable to Assess, with required rationale for each response. The Government will use all available information to assign an overall Past Performance rating of Acceptable or Unacceptable to determine if there is reasonable expectation the offeror will successfully perform the solicitation requirements.

DEPARTMENT OF THE ARMY
U.S. ARMY CONTRACTING COMMAND – ROCK ISLAND
3055 RODMAN AVENUE
ROCK ISLAND, IL 61299-8000
20 October 2026
SUBJECT: Offeror Past Performance Assessment in Support of Request for Proposal (RFP) Number W519TC-26-R-A076
Dear Sir / Ma’am, The US Army Contracting Command – Rock Island is currently conducting a competitive source selection to evaluate Offerors on the subject RFP. As part of this evaluation, we have requested that the Offerors provide information about their past performance on recent and relevant federal, state, or local Government and/or commercial contracts involving services performed under North American Industry Classification System (NAICS) 541612 or similar efforts. You have been identified as the point of contact for the enclosed past performance reference.
Your assessment of their performance is extremely valuable to our evaluation. The Government will consider the information provided in this questionnaire together with information obtained from other sources when evaluating the Offeror’s past performance. The Government will assign an overall Past Performance rating of Acceptable or Unacceptable based on whether the Government has a reasonable expectation that the Offeror will successfully perform the requirements of the solicitation. The questionnaire requests your assessment and supporting rationale; however, you are not being asked to assign the Government’s final Past Performance rating. Please provide factual, complete, and candid information based on firsthand knowledge of the contractor’s performance. If you do not have sufficient knowledge to provide a meaningful assessment, please identify that limitation in the questionnaire.
Please complete and return the enclosed questionnaire to the Contracting Officer at email: brooke.b.sheraden.civ@army.mil and Contract Specialist at email kynndel.m.wallarab.civ@army.mil, no later than fifteen (15) calendar days after receipt of request or DATE, whichever occurs first.
Your cooperation is greatly appreciated. Questions may be directed to the Contract Specialist and undersigned at the email addresses listed above.
Sincerely, Brooke B. Sheraden Contracting Officer, ACC-RI
Performance Assessment Questionnaire
Please provide your candid responses. The information that you provide will be used in the awarding of federal contracts. Therefore, it is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators. If you do not have knowledge of or experience with the company in question, please forward this Questionnaire to the person who does and notify the KO. Please return the completed Questionnaire to the KO identified in the cover letter within the stated timeframe.
Rating Definitions:
Acceptable: The available information indicates that the contractor’s performance met contractual requirements, or that any performance problems were isolated, adequately addressed, and supported by effective corrective actions. Based on the information available, the respondent has no basis to conclude that the contractor would be unable to successfully perform a similar effort.
Unacceptable: The available information indicates material performance problems, such as failure to meet contractual requirements, recurring schedule or technical deficiencies, ineffective corrective actions, termination, cure notice, show-cause letter, or other adverse performance information that causes the respondent to have no reasonable expectation that the contractor would successfully perform a similar effort.
Unable to Assess: The respondent does not have sufficient firsthand knowledge of the contractor’s performance to provide a meaningful assessment. This response is not a Government Past Performance rating. The Government will determine the Offeror’s overall Past Performance rating based on all available information.
