About this file
This file is a Subcontractor/Teaming Partner Consent Form for the HADES (Hybrid Architecture and Development for Experimental Systems) Services contract solicitation FA8809-26-R-B002. The form is issued by the Department of the Air Force, Space Systems Command, Innovation and Prototyping Delta, located at Kirtland Air Force Base, New Mexico, with an effective date of 21 July 2026.
The consent form authorizes the Government to disclose subcontractors' and teaming partners' past and present performance information to the prime contractor during the source selection process, including exchanges, discussions, and clarifications. This disclosure is necessary under the Privacy Act of 1974 to enable meaningful evaluation and dialogue between the Government and prime contractor. The form applies to all performance information from current and prior contracts and remains valid throughout the entire source selection process. Participation is voluntary; however, failure to provide consent may limit the Government's ability to conduct a complete past performance evaluation and could impact the source selection decision. Prime contractors are required to obtain completed, signed consent forms from each major subcontractor and teaming partner and submit them as part of the Past Performance Volume of their proposal. The point of contact for inquiries is Lt Col John Purcell (PCO) at Kirtland AFB, New Mexico.

Subcontractor/Teaming Partner Consent Form
Privacy Act Statement The information requested is authorized under 10 U.S.C. 3206. This information will be used to evaluate past (and present) performance of Offerors. Information will be used solely for source selection purposes and may be shared with the prime contractor/lead entity during exchanges and discussions. Providing this consent is voluntary; however, failure to provide consent may limit the Government's ability to fully assess the prime contractor's proposal and could impact the source selection decision.
(U) INSTRUCTIONS TO EVALUATORS
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent per the Privacy Act of 1974 (5 U.S.C. § 552a). Because a prime contractor is a private party, the Government requires this consent before disclosing subcontractor/teaming partner past and present performance information to the prime contractor during exchanges or discussions.
To assist the Government in assessing past performance relevancy and confidence, the Offeror shall obtain this completed consent form from each major subcontractor and teaming partner identified in the proposal.
Without this consent, the Government may be unable to discuss specific past performance concerns or strengths related to the subcontractor/teaming partner with the prime contractor, which may limit the Offeror's ability to address evaluation findings during exchanges.
Note to Offerors: This form should be provided to each subcontractor/teaming partner for their completion and signature. The completed, signed forms shall be collected by the Offeror and submitted as part of the Past Performance Volume of the proposal.
