About this file
This is a Sworn Statement Under Section 287.133(3)(a) of the Florida Statutes concerning public entity crimes, required for vendors bidding on contracts with Okaloosa County, Florida for the West Sunset Lift Station project. The document serves as a certification form that must be completed and notarized by any entity submitting a bid or proposal to the county. The sworn statement requires the submitting entity to declare its status regarding convictions for public entity crimes, which are defined as violations of state or federal law related to transactions with public entities, including offenses involving antitrust, fraud, theft, bribery, collusion, racketeering, conspiracy, or material misrepresentation occurring after July 1, 1989.
The document requires the bidder to indicate one of four compliance statuses: that neither the entity nor its officers, directors, executives, partners, shareholders, employees, members, or agents active in management have been convicted of a public entity crime; that a proceeding resulted in the entity not being placed on the convicted vendor list; that the entity was placed on the list but subsequently removed through an administrative hearing; or that no action has been taken by the Department of General Services. The form must be signed and notarized, with the notary public verifying the signatory's identity and administering the oath. This certification is a standard prerequisite requirement for participation in Florida public procurement processes and must be submitted with the bid for the West Sunset Lift Station project, which involves furnishing and installing a tremie style wetwell along with pumps and electrical systems to replace the existing lift station.
