About this file
This is an exhibit document establishing System for Award Management (SAM) registration requirements for the Board of County Commissioners of Okaloosa County, Florida's West Sunset Lift Station contract opportunity. The document outlines mandatory SAM database registration protocols that prospective offerors must complete prior to contract award, during performance, and through final payment. Offerors are required to register with all mandatory information including a unique entity identifier, Electronic Funds Transfer (EFT) indicator if applicable, Commercial and Government Entity (CAGE) code, and data required by the Federal Funding Accountability and Transparency Act of 2006. The registration must include completion of Core, Assertions, and Representations and Certifications sections, as well as Points of Contact information, with Government validation of all mandatory data fields including Taxpayer Identification Number (TIN) verification with the Internal Revenue Service.
Offerors must enter their unique entity identifier and EFT indicator on the cover page of their offer and understand that failure to register within the timeframe prescribed by the Contracting Officer will result in award proceeding to the next otherwise successful registered offeror. The registration process typically requires 48 hours for completion; therefore, offerors are advised to apply for registration immediately upon receipt of the solicitation. Prospective contractors lacking a unique entity identifier should contact the designated entity at www.sam.gov to obtain one and prepare required documentation including company legal business name, physical and mailing addresses, telephone number, employee count, business start date, chief executive officer information, line of business, and company headquarters details. Additional information on registration procedures is available through https://www.acquisition.gov.
