About this file
This is a Subcontractor Summary Spreadsheet issued by the City of St. Petersburg, Florida, which serves as an instructional document and worksheet that prime contractors must complete and submit with their bids for City projects. The worksheet requires prime contractors to identify all intended subcontractors, including their names, business addresses, contact information, trade classifications, and subcontract amounts. Contractors must indicate whether subcontractors are Small Business Enterprises (SBEs) or Minority/Women Business Enterprises (MWBEs) and specify whether they are first or second-tier subcontractors. The document establishes that subcontractors cannot be changed or substituted after a prime contractor is awarded the project and a pre-construction meeting is held, with limited exceptions including failure to execute a contract, lack of appropriate licensing, insolvency, ineligibility, performance failure, work quality deficiencies, or substantial project delays. If SBE subcontractors are used to meet percentage requirements, any replacements must also be SBEs unless the prime contractor demonstrates alternative compliance.
The spreadsheet requires contractors to document the percentage of contract work that may be subcontracted and track additions and deletions of subcontractors with corresponding reason codes. Any changes to the subcontractor list after award must be submitted in writing to the City's Procurement & Supply Management office, specifically to the Contracts Compliance Manager. The instructions note that restrictions on subcontracting percentages and multi-tier subcontractor use do not apply to Florida Department of Transportation (FDOT) Local Agency Program (LAP) or federally funded projects. This worksheet template includes fields for City approval signatures and dates, with a revision date of January 2020.
