U.S. Department of Justice United States Marshals Service Court Security Officer Weapons Qualification Record INSTRUCTIONS: U.S. Marshals Service Court Security Officers (CSO) are required to be qualified to operate a service weapon. Form CSO-014 should be completed by the contractor's authorized Firearms Instructor and returned to the CSO's district contracting officer's representative (COR) upon completion of annual weapons qualification, or as required for new hires.

USE OF DEADLY FORCE AND FIREARMS POLICY: I have read and understand the current DOJ Uniform Deadly Force Policy as well as the USMS contract requirements.
CONTRACTOR'S AUTHORIZED FIREARMS INSTRUCTOR CERTIFICATION: This certifies that the course of fire, qualification level, scores, weapons and ammunition used are authorized and indicated herein, and that the individual stated above demonstrated proficiency with their issued weapon.
19. The firearm described herein has been inspected by the contractor's authorized Firearms Instructor (named in Block 18) and meet USMS contract requirements for serviceability.
20. The holsters and accessory equipment used (magazines/magazine holders) have also been inspected and meet USMS contract requirements for design and serviceability.
21. The body armor described herein has been inspected by the contractor's authorized Firearms Instructor (named in Block 18) and meet USMS contract requirements for serviceability.
