About this file
This document is a Safe Practices Manual Review Checklist used by the U.S. Army Corps of Engineers (USACE) to evaluate dive contractor safety packages for federal construction projects.
The checklist establishes 24 minimum requirements that dive contractors must submit for USACE approval, organized according to EM 385-1-1 Section 30-7.b standards. Required submissions include dive safety procedures and checklists; assignments and responsibilities of dive team members; equipment certifications, procedures, and inspection checklists; and comprehensive emergency procedures addressing entrapped or fouled divers, loss of vital support equipment, loss of gas supply, loss of communication, lost diver scenarios, injured diver plans, fire discovery, diver loss of consciousness, rapid ascent, and surface crew illness or injury. Contractors must also provide appropriate U.S. Navy decompression tables (including No Decompression Limits and Repetitive Group Designation tables, Residual Nitrogen Timetables, and Standard Air Decompression Tables); procedures for internal safety inspections with specified frequencies and checklists; sample diving log sheets and repetitive dive worksheets; a complete copy of OSHA 29 CFR 1910 Subpart T with the employer's compliance policy statement; outlines of fitness for duty and medical requirements; and administrative and recordkeeping procedures. The checklist requires contractors to provide project name, contract number, dive contractor information, prime contractor information, and submittal details. USACE reviewers mark each requirement as "Accepted," "Possible Action Needed Before Acceptance Can Be Given," or "Not Acceptable resubmit." Incomplete submissions may result in rejection or process delays. The checklist was last updated on July 23, 2026.
