About this file
This document comprises a collection of USACE (U.S. Army Corps of Engineers) dive safety review checklists and certification forms used to evaluate contractor dive operations on federal projects. The checklists are organized around four primary submission packages that contractors must complete and submit for USACE approval: Safe Practices Manual, Dive Operations Plan, Activity Hazards Analysis, and Emergency Management Plan, along with Dive Personnel Qualifications documentation.
The Safe Practices Manual checklist requires contractors to provide 11 minimum items including dive safety procedures, equipment certifications, emergency procedures for various scenarios (lost diver, fouled diver, fire, equipment failure, medical injury), decompression tables, diving logs, repetitive dive worksheets, fitness-for-duty requirements, and administrative recordkeeping procedures, all referencing EM 385-1-1 Section 30-7.b. The Dive Operations Plan checklist mandates 17 items such as mission dates/times/location, diving mode and methodology, maximum bottom times, equipment lists, dive platform type, surface and underwater condition assessments, topside support identification, communication means, and inclusion of a required statement regarding plan alterations requiring Dive Dive Coordinator (DDC) approval. The Activity Hazards Analysis checklist requires development following EM 385-1-1 paragraphs 1-6, 2-6, or 12-6 as applicable, including activity location details, task steps, hazard identification, control measures, Risk Assessment Code (RAC) assignment, Hazardous Energy Control procedures where appropriate, and clearances/permits. The Emergency Management Plan checklist specifies six items covering recompression chamber and hospital locations with contact information, USCG Rescue Coordination Center details, emergency victim transport procedures, emergency service activation protocols, and diver rescue procedures with team member responsibilities. Additionally, the document includes a Dive Personnel Qualifications checklist requiring documentation of diver certifications, training experience, first aid/CPR/oxygen certifications, dive physicals, and rigger certifications, plus a Surface Supplied Air Pre-Dive Equipment Certificates form for tracking serial numbers and maintenance dates for umbilicals, deck whips, gauges, recompression chambers, compressors, volume tanks, air certificates, dive helmets, and bailout bottles. Incomplete submissions may result in rejection or process delays.
