About this file

This document is a Past Performance Questionnaire template used to evaluate contractor performance on federal contracts. The form is divided into two primary sections: contractor-completed blocks and client-completed blocks, along with a comprehensive performance evaluation section.

The contractor completes Blocks 1-4, providing firm information (name, CAGE code, DUNs number, contact details), work performed capacity (prime contractor, subcontractor, joint venture, or other), contract details (contract number, type, award and completion dates, original and final pricing), and project description including complexity level and relevance to the current solicitation. The client completes Blocks 5-8, documenting their name, title, contact information, role in the project, questionnaire completion date, and signature. Section D contains the core performance evaluation using a six-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Available (N). Evaluators must provide narrative explanations for any Unsatisfactory or Marginal ratings.

Performance is assessed across seven major categories: Quality (technical data preparation, quality standards, problem resolution, and quality control); Schedule/Timeliness (delivery compliance and resource utilization); Customer Satisfaction (end-user satisfaction, contractor cooperation, and overall satisfaction); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, requirement changes, and overall management effectiveness); Cost/Financial Management (adherence to contractual pricing, innovative cost-reduction proposals, invoice accuracy, accounting system adequacy, contract termination history, and financial stability); Safety/Security (safety environment maintenance and security compliance); and General (emergency response capability, contractual compliance, future engagement likelihood, and overall work performance rating). The form concludes with narrative sections for documenting strengths, weaknesses, deficiencies, and general comments regarding performance risk evaluation.
