About this file

This is a Request for Quotation (RFQ) issued by the 375th Contracting Squadron at Scott Air Force Base, Illinois, for full-service maintenance, repair, testing, and inspection of thirty-two (32) Vertical Transportation Equipment (VTE) units across the installation. The RFQ, numbered FA440726QDW01, is a 100% Total Small Business Set-Aside under NAICS code 238290 (Other Building Equipment Contractors) with a $22.0 million small business size standard. The scope encompasses routine preventive maintenance, unscheduled emergency repairs, and mandatory safety testing for passenger elevators, freight elevators, and wheelchair lifts to ensure compliance with ASME A17.1 safety codes. The contract vehicle is a single Requirements Contract with a five-year ordering period (October 1, 2026 through September 30, 2031), followed by an initial Firm-Fixed-Price Task Order for the Base Year plus four Option Years covering routine maintenance. Additional over-and-above repairs and parts will be executed through separate FFP Task Orders issued as needed. The deadline for quote submission is July 14, 2026 at 9:00 AM CST, with an optional site visit scheduled for June 25, 2026 at 9:00 AM CST.

Offerors must submit pricing for five separate twelve-month periods (Base Year and four Option Years) and are required to submit four volumes: (1) Administrative/Cover Letter addressing organizational conflict of interest, payment terms, and performance dates; (2) Technical Proposal (maximum 10 pages) addressing technical plan, emergency response strategy, personnel qualifications, and program management submittals demonstrating the ability to maintain a 98% overall VTE availability rate and meet strict response times (24 hours routine, 2 hours on-duty emergency, 3 hours off-duty emergency); (3) Past Performance Information with three relevant examples from the past five years; and (4) Pricing Worksheet with unit and total prices for all CLINs. All proposed pricing must be inclusive of labor, specialized tools, diagnostic equipment, materials, transportation, and overhead. Evaluation will be conducted sequentially on a best value tradeoff basis, with Technical acceptability evaluated first on an Acceptable/Unacceptable basis, followed by Past Performance evaluation (which is significantly more important than Technical and Price combined), and then Price evaluation for reasonableness, completeness, and balance. The Government reserves the right to award to a higher-priced, higher-rated offeror based on superior past performance.

RFO 52.212-1 Instructions to Offerors-Commercial Products and Commercial Services

(a) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. As a minimum, offers shall include—

(1) The solicitation number;
(2) The name, address, telephone number of the Offeror;
(3) The Offeror’s Unique Entity Identifier (UEI) and, if applicable, Electronic Funds Transfer (EFT) indicator;
(4) Information necessary to evaluate the factors contained in the provision at 52.212-2 or as described in the solicitation;
(5) Responses to provisions that require Offeror completion of information, representations, and certifications (other than those collected via the System for Award Management (SAM)); and
(6) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation and any solicitation amendments.

(b) Period for acceptance of offers. The Offeror agrees to hold the prices in its offer firm until 1 October 2026 unless another time period is specified in an addendum to the solicitation.

(c) Late submissions, modifications, revisions, and withdrawals of offers.

(1) Offerors are responsible for submitting offers and any modifications or revisions to the Government office designated in the solicitation by the time specified in the solicitation.
(2) Any offer, modification, or revision received after the time specified for receipt of offers is “late” and will not be considered unless it is received before award is made and the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition. However, a late modification of an otherwise successful offer that makes its terms more favorable to the Government will be considered at any time it is received and may be accepted.
(3) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(4) Offerors may withdraw their offers by written notice to the Government received at any time before award.

(d) Contract award. The Government intends to evaluate offers and award a contract without interchanges with Offerors. Therefore, the Offeror’s initial offer should contain the Offeror’s best terms. However, the Government reserves the right to conduct interchanges, if necessary. The Government may reject any or all offers if such action is in the public interest, accept other than the lowest offer, and waive informalities and minor irregularities in offers received.

(e) Debriefings. If a post award debriefing is given to requesting Offerors, the Government will disclose the following information, if applicable:

(1) The agency’s evaluation of the significant weak or deficient factors in the debriefed Offeror’s offer.
(2) The overall evaluated cost or price and technical rating of the successful Offeror and the debriefed Offeror and past performance information on the debriefed Offeror.
(3) The overall ranking of all Offerors when any ranking was developed by the agency during source selection.
(4) A summary of the rationale for award.
(5) For acquisitions of commercial products, the make and model of the product to be delivered by the successful Offeror.
(6) Reasonable responses to relevant questions posed by the debriefed Offeror as to whether the agency followed source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities.

(End of provision)

ADDENDUM TO RFO 52.212-1, INSTRUCTIONS TO OFFERORS—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES

The following is tailored to this acquisition and is hereby added via addendum. To assure timely and equitable evaluation of the quote, the offeror shall follow the instructions contained herein. The offeror is required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offeror being ineligible for award. The offeror must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale in the cover letter.

1. GENERAL AND SUBMISSION INSTRUCTIONS

All questions regarding this RFQ must first be directed to the Contract Specialist, Nicholas “Dillon” Weiss, via e-mail at nicholas.weiss.5@us.af.mil, with the Contracting Officer, Brandon Katka, CC’d at brandon.katka@us.af.mil . The deadline for questions is 02 July 2026 at 9:00 AM.

Your email submission must be checked and determined to be "virus free" prior to submission. After receipt of your quote, the Contract Specialist or Contracting Officer will email a confirmation of receipt. If you do not receive a confirmation of receipt prior to the due date and time, you are encouraged to contact the CS and CO to ensure the quote was received.

This acquisition does not require certified cost or pricing data. However, if deemed necessary, the Contracting Officer may request other than certified cost and pricing data to make a price fair and reasonable determination. To avoid delays, the offeror is encouraged to submit documentation deemed appropriate to support the proposed solution and pricing.

Organizational Conflict of Interest (OCI): At the time of submitting a quote, the offeror must state affirmatively whether or not it has OCI issues and must submit an OCI Identification and Mitigation Plan with the quote if OCI issues exist. The Government-accepted OCI Identification and Mitigation Plan shall be incorporated by reference into any resultant contract. If no OCI exists, the offeror shall provide a statement stating no OCI exists.

SPECIFIC INSTRUCTIONS FOR QUOTE SUBMISSION

The offeror is required to submit the following volumes in response to this solicitation:

VOLUME 1 – ADMINISTRATIVE (COVER LETTER)

The Cover Letter shall include the Offeror’s points of contact with e-mail addresses and phone numbers who are authorized to contractually obligate the company, CAGE Code, UEI Number, and large/small business status/categories.

The Cover Letter must contain a statement that the company understands the requirements specified and will meet the performance standards and requirements therein.

The Cover Letter must also contain a statement that the company does or does not take exception to any of the requirements of this RFQ. Any exceptions shall be clearly outlined with supporting rationale.

Finally, the Cover Letter must provide the following administrative information:

· Expiration Date of Quote (must be valid through at least 1 October 2026)

· Proposed Payment Terms or Discount (i.e., Net 30, 1% 10 days)

· Performance Begin and Complete Dates

VOLUME 2 – FACTOR I: TECHNICAL PROPOSAL

Please provide a summary of your technical approach on how you will perform full maintenance, repair, and required inspections for the 32 Vertical Transportation Equipment (VTE) units at Scott AFB in accordance with the Performance Work Statement (PWS).

Offerors shall not simply copy and paste the information directly from the Statement of Work, but rather shall provide their proposed solution for accomplishing the required service.

The technical proposal should utilize Times New Roman font, 12 pt font sizing, with standard single spacing. The maximum length of the Technical proposal will be 10 pages. If the Technical proposal is over 10 pages, please contact the Contract Specialist and Contracting Officer.

The Technical Proposal must explicitly address the following elements:

· Technical Plan: In accordance with the PWS, the offeror shall submit a detailed technical plan describing their technical methodology and approach to performing all requirements of the SOW. This plan must explicitly demonstrate how the offeror will maintain the required minimum 98% overall availability rate for the 32 VTE units.

· Emergency Response Strategy: The offeror shall outline an adequate plan demonstrating their ability to meet the strict response times required by the PWS: 24 hours for routine service calls, 2 hours for on-duty emergency calls, and 3 hours for off-duty emergency calls.

· Personnel Qualifications: The offeror shall provide documentation demonstrating that all personnel performing VTE maintenance possess the necessary and current licenses and certifications required by the SOW for elevator/vertical transportation maintenance. Submit copies of the relevant certifications.

· Program Management Submittals: As required under the ASME A17.1 National Elevator Safety Code, the offeror shall provide a clear approach for how they will develop and execute the required Quality Control Program (QCP) and equipment-specific Maintenance Control Programs (MCPs) within 30 days of contract award as mandated by the PWS.

VOLUME 3 – FACTOR II: PAST PERFORMANCE

The offeror must complete the Attachment 2 Past Performance Information (PPI) Sheet. Provide three (3) past performance examples of recent and relevant work.

· Recent is defined as work performed within the past five (5) years.

· Relevant is defined as work involving the full maintenance, repair, and safety inspection/testing of Vertical Transportation Equipment (VTE) or elevators of similar scope, magnitude, and complexity to the 32 units specified in the PWS.

For each example, complete a Past Performance Information Sheet (PPIS), which should include:

· Customer name and contact information

· Dollar value of the work performed

· Contract number (if applicable)

· Period of performance

· Brief description of the work performed

· Customer assessment of performance (e.g., CPARS rating, letter of recommendation, and point of contact willing to be interviewed)

In addition to the PPIs, offerors may submit no more than one (1) page of additional information for each of the three (3) past performance examples submitted.

VOLUME 4 – FACTOR III: PRICE

The offeror shall submit pricing by completing the pricing tables provided within the solicitation package (the CLIN structure and Pricing Worksheet). The proposed pricing must support the completeness and reasonableness of the proposed prices for all contract line-item numbers (CLINs) for the Base Year (CLIN 0001) and all four Option Years (CLINs 1001, 2001, 3001, and 4001).

The pricing information submitted shall fully support the PWS requirements for each element specified. Information shall be segregated by the performance periods and the total price for each CLIN as specified in the solicitation. Information shall also show the calculated Grand Total (Total Evaluated Price) representing the sum of the base period and all four option periods combined.

The extended amounts must equal the whole dollar unit price multiplied by the number of units (i.e., 12 months). All unit and total dollar amounts provided shall be rounded to the nearest dollar. All proposed pricing shall be inclusive of all costs necessary to perform the services described in the Performance Work Statement (PWS), including labor, specialized tools, diagnostic equipment, materials, transportation, and overhead.

VOLUME 5 – SOLICITATION PROVISIONS AND CONTRACT CLAUSES

The offeror shall submit completed copies of all standard solicitation provisions and contract clauses that require representations, certifications, or fill-in information from the offeror, unless those representations and certifications are fully completed and up-to-date within the offeror's SAM.gov profile.

Addendum to 52.212-2 Evaluation-Commercial Items (NOV 2021) This is a best value source selection utilizing the subjective tradeoff process to determine the offeror who gives the Air Force the greatest confidence it will best meet, or exceed, the requirements as stated in this solicitation.

Award will be made to the offeror who is deemed responsible in accordance with the RFO, as supplemented, whose quote conforms to the solicitation's requirements (to include all stated terms, conditions, representations, certifications, and all other information required by the 52.212-1 addendum of this solicitation) and is judged, based on an integrated assessment of Technical, Past Performance, and Price, to represent the best value to the Government.

The Government seeks to award to the offeror who gives the Air Force the greatest confidence that it will best meet, or exceed, the requirements. This may result in an award to a higher rated, higher priced offeror, where the decision is consistent with the evaluation factors, and the requirement owner and Contracting Officer reasonably determine that the superior past performance outweighs the cost difference.

While the Government's evaluation team will strive for maximum objectivity, the tradeoff process, by its nature, is subjective; and therefore, professional judgment is implicit throughout the entire process.

The following factors will be used to evaluate the Vertical Transportation Equipment (VTE) Maintenance requirement:

FACTOR 1 – TECHNICAL

Measures of merit for this factor are evaluated on an Acceptable or Unacceptable basis. To be rated Acceptable, the technical approach must meet the minimum requirements for all of the following essential elements derived from the Performance Work Statement (PWS):

Technical Plan: A detailed methodology describing how the offeror will perform full maintenance, repair, and required safety inspections (including annual and 5-year tests) on the 32 VTE units at Scott AFB to maintain the required minimum 98% overall availability rate.

Emergency Response Strategy: An adequate plan demonstrating the ability to meet the strict PWS response times: 24 hours for routine service calls, 2 hours for on-duty emergency calls, and 3 hours for off-duty emergency calls.

Personnel Qualifications: Documentation demonstrating that all personnel performing VTE maintenance possess the necessary and current licenses and certifications required by the PWS for elevator maintenance.

Program Management Submittals: A clear approach for how the offeror will develop and execute the required Quality Control Program (QCP) and equipment-specific Maintenance Control Programs (MCPs).

FACTOR 2 – PAST PERFORMANCE

Offerors are evaluated on the recency, relevancy, and quality of their past performance to establish an overall reliability and confidence rating.

FACTOR 3 – PRICE

The Initial Total Evaluated Price (I-TEP) will be calculated as the sum of the Offeror’s proposed prices for the base period and four 12-month option periods on the initial task order.

STRENGTHS EXCEEDANCES

The Government intends, if it is in its best interest, to incorporate into the resulting contract all beneficial aspects of the awardee’s quote and all above-threshold (minimum) attributes, performance levels, or capabilities for which evaluation credit was given.

By submission of your quote, the offeror agrees to all solicitation requirements including terms and conditions and representations and certifications. The offeror shall clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The Government reserves the right to cancel this solicitation with no obligation to the offeror.

A. FACTOR 1: TECHNICAL EVALUATION

The Government will evaluate the Technical Proposal on an Acceptable/Unacceptable basis, assigning a rating of Acceptable or Unacceptable, as defined below:

Acceptable: The submitted proposal is compliant with RFQ and PWS requirements and meets all measures of merit listed under Factor 1.

Unacceptable: The submitted proposal is non-compliant with RFQ and PWS requirements, rendering it ineligible for award due to the presence of one or more deficiencies.

B. FACTOR 2: PAST PERFORMANCE EVALUATION

The Government will evaluate the offeror’s Past Performance to determine whether the offeror’s past performance is recent and relevant as defined in RFO 52.212-1. The Government will also evaluate how well the offeror performed on prior contracts/projects.

In addition to the submitted Past Performance Information Sheets, the government reserves the right to use other sources of past performance information in determining a reliability rating. These sources include but are not limited to: CPARS, internet searches, Better Business Bureau, Google and other online reviews, etc.

Step 1: Recency Assessment The Government will first review the past performance information sheets provided by the vendor and those found in other sources (e.g., CPARS) to determine if they are recent. A past performance reference is considered recent if the work was performed within five (5) years prior to the issuance date of this RFQ. References that are not recent will not be evaluated further.

Step 2: Relevancy Assessment For all recent past performance references, the Government will assess their relevance to the work required by the PWS. The Government will consider the scope, magnitude, and complexity of the prior work specifically as it relates to Vertical Transportation Equipment (VTE) and elevator maintenance. A relevancy rating of Highly Relevant, Relevant, or Not Relevant will be assigned.

Highly Relevant: The work performed was virtually the same as the PWS requirements for VTE Maintenance.

Relevant: The work performed was similar to the PWS requirements for VTE Maintenance.

Not Relevant: The work was not similar to the PWS requirements for VTE Maintenance.

Step 3: Performance Quality and Reliability Assessment The Government will evaluate the quality of performance on contracts deemed recent and relevant to assign an overall confidence rating as defined below:

High Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a high degree of confidence that the vendor will successfully perform the required effort. The vendor's record indicates performance was of high quality.

Adequate Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a reasonable expectation that the vendor will successfully perform the required effort. The vendor's record indicates performance was of acceptable quality.

Neutral Confidence: No recent and relevant performance record is available for review, or the record is so sparse that no meaningful confidence assessment can be made. This rating is not viewed favorably or unfavorably.

Low Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a low expectation that the vendor will be able to successfully perform the required effort. The record may indicate performance issues on one or more contracts.

C. FACTOR 3: PRICE EVALUATION

The Government will evaluate the quote submitted in response to this solicitation to ensure that the proposed price is complete, reasonable, and balanced. The Government reserves the right to reject an offer if the Contracting Officer determines that the proposed price does not meet the criteria of completeness, reasonableness, and balance.

Completeness: The offeror's price and other supporting information will be evaluated to ensure continuity and traceability of prices to the proposed technical plan and between initial quotes and any revisions thereto. The review will determine the adequacy of the offeror's quote in addressing and fulfilling the solicitation requirements.

Reasonableness: The quote will be reviewed to determine the reasonableness of price and the offeror's understanding of and ability to perform the requirement. Price reasonableness will be determined through the use of competition. In the event adequate price competition is not received, the Contracting Officer will use one or more of the price analysis techniques described at RFO 15.404-1.

Balance: In accordance with RFO 15.404, unbalanced pricing may increase performance risk and could result in payment of unreasonably high prices. All separately priced line items will be analyzed to determine if the prices are unbalanced. The offer may be rejected if the Contracting Officer determines the lack of balance poses an unacceptable risk to the Government.

The Initial Total Evaluated Price (I-TEP) will be calculated as the sum of the Offeror’s proposed prices for the base period and four 12-month option periods. Evaluation of options and the Extension of Services (EOS) shall not obligate the Government to exercise such options.

A six-month Extension of Services in accordance with RFO 52.217-8, “Option to Extend Services,” will be utilized if necessary. The six-month EOS is not to be considered part of option period pricing and will be evaluated using the option prices of the final period. After any interchanges or quote revisions, if necessary, the Government will evaluate the Final TEP (F-TEP).

GENERAL CONSIDERATIONS

The quote must comply in all material respects with the requirements of law, regulations, and conditions set forth in the solicitation. To be eligible for award, the offeror must be determined responsible as defined in RFO 9.104-1.

A written notice of award or acceptance of an offer, mailed, or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

The Government reserves the right to conduct interchanges with offerors. These interchanges may be used to clarify aspects of quotes, resolve minor deficiencies, or allow offerors to revise their quotes (including price). The Government is not obligated to conduct interchanges with all offerors and may award without conducting interchanges.

BASIS OF AWARD

Award will be made on a best value tradeoff basis. Award will be made IAW the procedures specified in this Request for Quote (RFQ). A contract may be awarded to the offeror who is deemed responsible IAW RFO Subpart 9.1, as supplemented, whose quote conforms to the solicitation requirements and is judged to represent the best value to the Government.

To ensure an efficient and logical evaluation process, the Government will evaluate quotes sequentially using the following phases:

Phase 1. Technical Evaluation:

All timely quotes will initially be evaluated for Technical acceptability (Factor 1). Technical will be evaluated on a strict Acceptable/Unacceptable basis per the criteria described in this addendum. If a quote is deemed Unacceptable and not easily made acceptable, the proposal will be ineligible for award and removed from further consideration.

Phase 2. Past Performance Evaluation:

The Government will evaluate the Past Performance Proposals (Factor 2) for all offerors who received an "Acceptable" rating in Phase 1. Past performance will be graded on the Performance Quality and Confidence criteria as described in this addendum.

Phase 3. Price Evaluation:

The Government will evaluate the Price Proposals (Factor 3) for all technically acceptable offerors remaining from Phase 2. Price will be evaluated for reasonableness, completeness, and balance as described in this addendum. The Government will determine prices fair and reasonable through the use of competition. In the event adequate price competition is not received, the Contracting Officer will use one or more of the price analysis techniques described at RFO 15.404-1 to ensure the price to the Government for the initial task order is fair and reasonable.

Phase 4. Tradeoff and Award Decision:

The Contracting Officer will make a final best value tradeoff decision among the fully evaluated offerors. In making this decision, Past Performance (Factor 2) is significantly more important than Technical (Factor 1) and Price (Factor 3) combined. The Government reserves the right to award to a higher rated, higher priced offeror where the decision is consistent with the evaluation factors outlined in the solicitation and the Government reasonably determines that the superior past performance of the higher priced offeror outweighs the cost difference. In the event multiple offerors receive equal past performance ratings (and are technically acceptable), the tradeoff will be made between those tied offerors, with the award decision representing the best value to the Government.

Ombudsman

(a)An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes.

(b)The ombudsman may refer the interested party to another official who can resolve the concern.

Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c)If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, Mr. Robert Surrey, 510 POW/MIA Drive, Scott AFB IL 62225-5022, 618-229-0182, robert.surrey.1@us.af.mil of AFICC/KM. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Center/MAJCOM/FLDCOM/DRU ombudsman level, may be brought by the interested party for further consideration to the Department of the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (571) 256-2395, facsimile number (571) 256-2431.

(d) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the contracting officer.

(End of clause)

Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

The contractor shall provide an after-hours contact number or after-hours email in the Emergency Mass Notification System (EMNS) for each of their personnel, whose normal place of duty is on a DoD installation or within a DoD facility. The contractor shall comply with any additional requirements in DAFMAN 10-206 for emergency operational reporting.
