About this file
This is a Past Performance Information Sheet template for offerors responding to an RFQ for elevator maintenance services at Scott Air Force Base. Offerors must submit three (3) past performance references that are both recent (performed within five years of the RFQ issuance date) and relevant (involving maintenance, repair, and installation of vertical transportation equipment). The template requires offerors to document project titles, contract numbers, performing company names and roles, customer agency information including points of contact, total contract values, periods of performance, and detailed narratives describing how each project relates to the RFQ's Performance Work Statement. Specifically, offerors must address the scope of barrier systems (or in this case, elevator systems), magnitude such as number of units and facility size, and complexity including unique installation challenges and system integrations.
The template also requires offerors to provide evidence of performance quality through available CPARS records with ratings in key evaluation areas such as Quality, Schedule, Cost Control, and Management, or alternatively through letters of recommendation or other customer assessments. Offerors must indicate whether points of contact are willing to participate in performance interviews or surveys and must grant the U.S. Government permission to contact these references to verify the information provided and inquire about performance on the cited contracts. This template is part of a competitive evaluation process where past performance will be assessed alongside technical acceptability and price in determining contract award.
