(4) Information necessary to evaluate the factors contained in the provision at 52.212-2 or as described in the solicitation;
(5) Responses to provisions that require Offeror completion of information, representations, and certifications (other than those collected via the System for Award Management (SAM)); and
(6) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation and any solicitation amendments.

(e) Debriefings. If a post award debriefing is given to requesting Offerors, the Government will disclose the following information, if applicable:
(1) The agency’s evaluation of the significant weak or deficient factors in the debriefed Offeror’s offer.

VOLUME 2 – FACTOR I: TECHNICAL PROPOSAL
Please provide a summary of your technical approach on how you will perform comprehensive facility management, operations, and maintenance services for Building 1990 at Scott AFB in accordance with the Performance Work Statement (PWS).
Offerors shall not simply copy and paste the information directly from the SOW, but rather shall provide their proposed solution and methodology for accomplishing the required service. Additionally, offerors shall indicate how their company plans to comply with the Limitations on Subcontracting, by indicating if they will self-perform 50% or more of the work or by providing their subcontractor.
The technical proposal should utilize Times New Roman font, 12 pt font sizing, with standard single spacing. The maximum length of the Technical proposal will be 10 pages. If the Technical proposal is over 10 pages, please contact the Contract Specialist and Contracting Officer.
The Technical Proposal must explicitly address the following elements:
Technical Plan: In accordance with the PWS, the offeror shall submit a detailed technical plan describing their technical methodology and approach to performing all requirements of the PWS. This plan must explicitly demonstrate how the offeror will maintain the facility's mechanical, electrical, plumbing, and structural systems to ensure continuous operational availability. Offerors shall indicate how their company plans to comply with the Limitations on Subcontracting, by indicating if they will self-perform 50% or more of the work or by providing their subcontractor.
· Emergency Response Strategy: The offeror shall outline an adequate plan demonstrating their ability to meet the strict response times required by the PWS for demand maintenance, specifically Priority 1 (Emergency), Priority 2 (Urgent), and Priority 3 (Routine) service calls.
· Personnel Qualifications: The offeror shall provide documentation demonstrating that all personnel performing maintenance possess the necessary and current licenses and certifications required by the PWS for the specialized trades (e.g., HVAC, electrical, plumbing). Submit copies of the relevant certifications.
· Program Management Submittals: As required by the PWS, the offeror shall provide a clear approach for how they will develop and execute the required Management Plan, Quality Control Plan, and Safety Plan, ensuring compliance with standard commercial building codes, OSHA regulations, and environmental rules (e.g., EPA Section 608).
VOLUME 3 – FACTOR II: PAST PERFORMANCE
The offeror must complete the Attachment 4 Past Performance Information (PPI)/Questionnaire Sheets. Provide three (3) past performance examples of recent and relevant work.
· Recent is defined as work performed within the past five (5) years.
· Relevant is defined as work involving comprehensive facility management, preventative and demand maintenance, and management of multiple skilled trades (e.g., HVAC, electrical, plumbing) of similar scope, magnitude, and complexity to this requirement.
For each example, complete a Past Performance Information Sheet (PPIS), which should include:
· Customer name and contact information
· Dollar value of the work performed
· Contract number (if applicable)
· Period of performance
· Brief description of the work performed
· Customer assessment of performance (e.g., CPARS rating, letter of recommendation, and point of contact willing to be interviewed)
In addition to the PPIs, offerors may submit no more than one (1) page of additional information for each of the three (3) past performance examples submitted.
VOLUME 4 – FACTOR III: PRICE
The offeror shall submit pricing by completing the pricing tables provided within the solicitation package (the CLIN structure and Pricing Worksheet). The proposed pricing must support the completeness and reasonableness of the proposed prices for all contract line-item numbers (CLINs) for the Base Year and all four Option Years.
The pricing information submitted shall fully support the PWS requirements for each element specified. Information shall be segregated by the performance periods and the total price for each CLIN as specified in the solicitation. Information shall also show the calculated Grand Total (Total Evaluated Price) representing the sum of the base period and all four option periods combined.
The extended amounts must equal the whole dollar unit price multiplied by the number of units (i.e., 12 months). All unit and total dollar amounts provided shall be rounded to the nearest dollar. All proposed pricing shall be inclusive of all costs necessary to perform the services described in the Performance Work Statement (PWS), including labor, specialized tools, diagnostic equipment, materials, transportation, and overhead.
VOLUME 5 – SOLICITATION PROVISIONS AND CONTRACT CLAUSES
The offeror shall submit completed copies of all standard solicitation provisions and contract clauses that require representations, certifications, or fill-in information from the offeror, unless those representations and certifications are fully completed and up-to-date within the offeror's SAM.gov profile.
Addendum to 52.212-2 Evaluation-Commercial Items (NOV 2021) Basis of Award:
Award will be made IAW commercial procedures (RFO Part 12) and the procedures specified in the Request for Quote (RFQ). The Government intends to award a single Firm-Fixed-Price contract to the responsible offeror (IAW RFO Subpart 9.1) whose proposal conforms to the solicitation's requirements and is judged to represent the best value to the Government.
This evaluation will be conducted using a phased approach.
· Phase 1: Evaluation of Technical Approach
· Phase 2: Evaluation of Past Performance and Price.
Note: If a contractor’s proposal receives a rating of Unacceptable on the Technical Approach Factor in Phase 1, their proposal will not be evaluated further in Phase 2, and they will be considered ineligible for award.
Evaluation Factors:
A. Factor 1 – Technical Approach (Phase 1):
The Government will evaluate the Technical Proposal on an Acceptable/Unacceptable basis. Offerors will be evaluated on their understanding of the requirement and the feasibility of their proposed solution.
· Acceptable: The submitted proposal is compliant with RFQ and SOW requirements.
· Unacceptable: The submitted proposal is non-compliant with RFQ and SOW requirements, rendering it ineligible for award due to the presence of deficiencies.
B. Factor 2 – Past Performance (Phase 2):
For all technically acceptable proposals, the Government will conduct a performance confidence assessment based on the recency, relevancy, and quality of the vendor's past performance.
In addition to the submitted Past Performance Information Sheets, the government reserves the right to use other sources of past performance information in determining a reliability rating. These sources include but are not limited to: CPARS, internet searches, Better Business Bureau, Google and other online reviews, etc.
· Step 1: Recency Assessment. A past performance reference is considered recent if the work was performed within five (5) years prior to the issuance date of this RFQ.
· Step 2: Relevancy Assessment. The Government will assess relevance based on scope, magnitude, and complexity compared to the SOW.
· Highly Relevant: The work performed was virtually the same as the SOW requirements.
· Relevant: The work performed was similar to the SOW requirements.
· Not Relevant: The work was not similar.
· Step 3: Performance Quality and Confidence Assessment. The Government will assign an overall confidence rating:
· High Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a high degree of confidence that the vendor will successfully perform the required effort. The vendor's record indicates performance was of high quality.
· Adequate Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a reasonable expectation that the vendor will successfully perform the required effort. The vendor's record indicates performance was of acceptable quality.
· Neutral Confidence: No recent and relevant performance record is available for review, or the record is so sparse that no meaningful confidence assessment can be made. This rating is not viewed favorably or unfavorably.
· Low Confidence: Based on the vendor’s performance quality on recent and relevant efforts, the Government has a low expectation that the vendor will be able to successfully perform the required effort. The record may indicate performance issues on one or more contracts.
C. Factor 3 – Price (Phase 2):
The Final Total Evaluated Price (F-TEP) will be calculated as the sum of the Offeror’s proposed prices for the base period and four 12-month option periods, plus the Government-calculated six-month Extension of Services (EOS). Offerors shall only submit pricing for the base period and the four option periods; offerors shall not submit pricing for the six-month extension of services under RFO 52.217-8. For evaluation purposes, the Government will calculate the EOS price internally by multiplying the Offeror's proposed monthly cost of Option Year 4 by six (IAW RFO 52.217-8). Evaluation of options does not obligate the Government to exercise them. The Government reserves the right to reject an offer if the Contracting Officer determines that the proposed price does not meet the criteria of completeness, reasonableness, and balance.
The Government will evaluate the price proposal to ensure it is:
· Completeness: The offeror's price and other supporting information will be evaluated to ensure continuity and traceability of prices to the proposed technical plan and between initial quotes and any revisions to it. The review will determine the adequacy of the offeror's quote in addressing and fulfilling the solicitation requirements.
· Reasonableness: The quote will be reviewed to determine the reasonableness of price and the offeror's understanding of and ability to perform the requirement. Price reasonableness will be determined through the use of competition. In the event adequate price competition is not received, the Contracting Officer will use one or more of the price analysis techniques described at RFO 15.404-1.
· Balance: In accordance with RFO 15.404, unbalanced pricing may increase performance risk and could result in payment of unreasonably high prices. All separately priced line items will be analyzed to determine if the prices are unbalanced. The offer may be rejected if the Contracting Officer determines the lack of balance poses an unacceptable risk to the Government.
Tradeoff Methodology:
Award will be made on a best-value tradeoff basis. The non-price factor of Past Performance is significantly more important than the Technical Approach (must be acceptable) and Price factors. That said, Technically Acceptable Quotes will be the only quotes that move to Phase 2. The Government reserves the right to award to a higher-rated, higher-priced offeror where the past performance advantages offered are determined to be worth the additional price. A contract will be awarded to the offeror who is deemed responsible IAW RFO Subpart 9.1, whose proposal conforms to the solicitation requirements, and is judged to represent the best value to the Government. In the event that multiple offerors receive equal past performance ratings, price will be the determining factor.
General Considerations & Interchanges:
The proposal must comply in all material respects with the requirements of law, regulations, and conditions set forth in the solicitation. To be eligible for award, the offeror must be determined responsible as defined in RFO 9.104-1. A written notice of award or acceptance of an offer furnished to the successful offeror shall result in a binding contract.
The Government reserves the right to conduct interchanges with offerors to clarify aspects of proposals, resolve minor deficiencies, or allow revisions (including price). The Government is not obligated to conduct interchanges and may award without them.
Ombudsman
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes.
(b)The ombudsman may refer the interested party to another official who can resolve the concern.
Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
(c)If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, Lisa Gross, 510 POW/MIA Drive, Scott AFB IL 62225-5022, 618-229-0182, lisa.gross@us.af.mil of AFICC/KM. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Center/MAJCOM/FLDCOM/DRU ombudsman level, may be brought by the interested party for further consideration to the Department of the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (571) 256-2395, facsimile number (571) 256-2431.
(d) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the contracting officer.
(End of clause)
