About this file
This is a Past Performance Information Sheet template that requires offerors to submit three recent and relevant past performance references as part of their proposal for facility management services.
Offerors must complete one sheet for each reference, documenting projects performed within the five years preceding the RFQ issuance date. References must demonstrate comprehensive facility management, operations, and maintenance services for facilities of similar size, scope, and complexity, including preventative maintenance, demand repairs, and management of multiple skilled trades such as HVAC, electrical, and plumbing. The sheet requires offerors to provide general project information (title, contract/order number, performing company status), customer/agency details with point of contact information and willingness to participate in performance interviews, contract specifics (total value and period of performance), and a detailed narrative describing work performed with explicit connections to the solicitation's Performance Work Statement requirements. Offerors must address project scope, magnitude (facility square footage and major systems maintained), and complexity (including emergency service call management, hybrid FFP/T&M contract experience, and federal building code compliance). Evidence of performance quality must be provided through available CPARS ratings, letters of recommendation, or other customer assessments. By submitting this information, offerors grant the U.S. Government permission to contact references to verify performance claims.
