About this file

This file is an email chain documenting the insurance verification process for RFQ 250390 between the Orange County Transportation Authority (OCTA) and internal stakeholders Andrew Coppolo (Senior Contract Administrator) and Regie Peaslee (Senior Claims Representative, Risk Management). The RFQ concerns the removal and recycling/disposal of universal waste with a budget of $65,100 for a three-year term. Written questions or clarifications must be submitted no later than Tuesday, July 7, 2026, with OCTA providing written responses via addendum by Tuesday, July 14, 2026. The contract will be awarded to the lowest, responsive, and responsible firm on a time and expense basis. All firms must quote all line items in the pricing proposal to be considered responsive, and the successful contractor must comply with all applicable Equal Employment Opportunity Laws and Regulations.

The email chain specifically addresses insurance requirements and coverage provisions for the contract. Peaslee requested that Coppolo review the Insurance Indemnification Memo and confirm whether the previously used insurance requirements remained appropriate for this procurement. Key discussion points include whether the contractor is an existing vendor with prior experience performing this work for OCTA, clarification on what constitutes "necessary certificates for transportation, disposal, recycling and destruction" referenced in the Scope of Work documentation requirements, and the addition of Pollution Liability Insurance as a required coverage given the nature of universal waste handling, transportation, and disposal. Coppolo subsequently updated the Insurance Indemnification Memo to include Pollution Liability as item #5, which Peaslee reviewed and approved on June 3, 2026, authorizing continuation of the procurement process.

From: Regie Peaslee To: Andrew Coppolo Subject: RE: Review for Insurance for RFQ 250390 Removal and Recycling/Disposal of Universal Waste: Risk Review Date: Wednesday, June 3, 2026 9:30:29 AM

Good Morning Andrew:

Thanks for the updated Insurance Requirements Memo!

I have reviewed the language and coverages, and I agree. Please continue!

Additionally, by any chance do you know Carolina Coppolo from LA County Metro?

Thanks again, Regie Peaslee Senior Claims Representative, Risk Management Orange County Transportation Authority Tel # 714-560-5851 Rpeaslee@octa.net

Hi Regie, I have updated the Insurance Indemnification Memo (attached) to include item #5, Pollution Liability. Can you please review and advise if these are acceptable?

Thanks, -AC

Good Afternoon Andrew:

In addition to the questions below, given the type of waste to be handled, transported and disposed, it would be in OCTA’s best interest to include Pollution Liability Insurance. Please review with CAMM for OCTA’s standard required coverage for Pollution Liability Insurance.

Good Afternoon Andrew:

Thank you for the SOW, PO and the Insurance Requirement Memo.

Please advise, if this is an existing contractor and contract (in other words, has the contractor done this type of work for OCTA prior to this PO)? I am curious about the following Section in the SOW:

DOCUMENTATION:

CONTRACTOR shall supply OCTA with all necessary certificates for transportation, disposal, recycling and destruction, and all other verifications (bills of lading, receipts, waste reports, etc.) as deemed necessary for all waste streams removed as requested by the OCTA.

Do you know (or perhaps the PM knows) what the definition of “…all necessary certificates for transportation, disposal, recycling and destruction…” mean. Is the PM requesting that the contractor have a specialized certificate to transport, dispose of, recycle and destroy listed waste.

Please advise.

Good Afternoon Regie, Can you please review the attached and advise if okay to proceed with these insurance requirements? FYI they are the exact same as the insurance requirements used in the previous purchase order. Please let me know, however, if there are any updates to the language/requirements and/or if you need anything else for your review.
