About this file

This is a Proposer Information and Certification Sheet for a pressure washing services contract with the State of Oregon, Department of Administrative Services (DAS), State Procurement Services (SPS). The RFP (S-10700-00015314) seeks pressure washing services for state-owned buildings. The document serves as the certification form that proposers must complete and submit as part of their proposal response, establishing that proposers understand and accept all RFP requirements and agree to be bound by the contract terms and conditions outlined in Attachment A, as modified by any addenda. Proposers must acknowledge receipt of all addenda and confirm that their proposal constitutes a firm offer valid for 180 days following the closing date. If awarded, proposers agree to perform the scope of work and meet performance standards as defined in the final negotiated contract scope of work.

The certification sheet requires proposers to make multiple attestations regarding tax compliance, non-discrimination practices, sexual harassment prevention policies (mandatory for contracts with an anticipated value of $150,000 or more), wage equity compliance under ORS 652.220, broadband service compliance where applicable, and verification that neither the proposer nor its employees, agents, or subcontractors appear on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management. Proposers must also certify the absence of conflicts of interest, confirm the truthfulness and accuracy of all proposal contents, attest that proposals were prepared independently without collusion or fraud, and acknowledge that false or fraudulent statements may constitute claims under the Oregon False Claims Act with associated liabilities and penalties. This certification is required in addition to any certifications specified in the contract and Statement of Work at the time of contract execution.
