PAST PERFORMANCE QUESTIONNAIRE INSTRUCTIONS

(Updated May, 2018)

Sections 1-3: Provide for contract-related descriptive information and identification of the evaluator.

Section 4: Lists the major work elements within our Performance Work Statement (PWS). Please provide your assessment of the “relevant experience” and “performance” associated with our PWS evidenced within the contract for which you are a reference. “Experience” means a contractor who has performed work in the specific PWS Element. “Did not Perform” means that the specific PWS element was not performed under the contract.

Section 5: Evaluates the contractor’s technical, schedule, and cost performance and management (Additional pages may be used for comments if desired). If you cannot answer any questions, please circle “N/R” for Not Rated.

The following definitions are offered for your use in assigning a performance rating for each of the factors in Sections 5 and 6:

Very High

(VH)

The Offeror’s relevant past performance is of exceptional merit; indicating exemplary performance in a timely, efficient, and economical manner; very minor (if any) problems with no adverse effect on overall performance.

High

(H)

The Offeror’s relevant past performance demonstrates very effective performance that would be fully responsive to contract requirements with contract requirements accomplished in a timely, efficient, and economical manner for the most part with only minor problems with little identifiable effect on overall performance.

Moderate

(M)

The Offeror’s relevant past performance demonstrates effective performance; fully responsive to contract requirements; reportable problems, but with little identifiable effect on overall performance.

Low

(L)

The Offeror’s relevant past performance meets or slightly exceeds minimum acceptable standards; adequate results; reportable problems with identifiable, but not substantial, effects on overall performance.

Very Low

(VL)

The Offeror’s relevant past performance does not meet minimum acceptable standards in one or more areas; remedial action required in one or more areas; problems in one or more areas which, adversely affect overall performance.

3.2 EMERGENCY OPERATIONS Utilization of National Incident Management System (NIMS): What level of NIMS was used and performance at each level. Security Police Supervisors, Emergency Managers, Security Police Officers, Emergency Medical Dispatcher (EMD), and other first responders.

3.4 TEST, TRAINING & EXERCISE (TT&E) Effectiveness in providing for the developing, maintaining, and administering a comprehensive Test, Training, and Exercise (TT&E) program in accordance with standards and requirements outlined in National Incident Management System (NIMS), Federal Continuity Directive-1 (FCD-1), the Homeland Security Exercise and Evaluation Program (HSEEP), participates in Statewide weather drills, and NPR 8715.2B. Implement applicable emergency preparedness policies and directives, Federal, state, and local directives. Provide for an integrated and coordinated Federal, state, and local response consistent with the NIMS.

4.0 PROTECTIVE SERVICES COMMUNICATION CENTER (PSCC) Monitor and Maintain computer system security support to operate ACS/IDS/NVS/Duress electronic/Information Technology systems, including 911 dispatch capabilities and security alarm monitoring, used to control access to installations, areas, and facilities. Operate a Protective Services Communication Center (PSCC) emergency dispatch and 911 call center containing emergency and administrative telephones, point-to-point telephones, 2-way radio base stations with multi-net/bridging capabilities and all Life Safety and Security Systems, voice recording systems. Perform emergency dispatch functions and procedures: medical, fire, security, and local hazard alarm operator; emergency (911) and non-emergency telephone operator, emergency and non-emergency text and paging systems operator, conference call operator; protective services radio systems operator, Telephone Device for the Deaf (TDD) operator, and intercom operator.

5.3 ELECTRONIC SECURITY SYSTEM SERVICES Effectiveness in providing support for security related electrical services, integration, construction, planning, design, installation, and maintenance of enterprise Electronic Security Systems (ESS). Install, maintain, test, and document ESS for intrusion alarms, network video systems, access control, electronic locking devices, active bollards and other security barriers (including turnstiles), audio alarms, body cameras, and mobile patrol video cameras and recording equipment.

6.1 SECURITY AND SECURITY POLICE SERVICES Ability to provide crime detection and prevention, make arrests, respond to emergencies, and provide roadway and parking area safety, to include speed control with radar. The ability to apply Federal Rules of Evidence, crime scene protection, and evidence preservation.

6.2 TRAFFIC ENFORCEMENT Effectiveness in traffic accident response and investigation. Investigate traffic incidents and offenses, safeguard accident scenes, enforce traffic laws and regulations, direct and control traffic, and issue citations.

7.0 IDENTITY CREDENTIAL AND ACCESS MANAGEMENT (ICAM) Ability to provide ID Management and badging operations used to identify and badge employees and visitors. Utilize systems and related databases (e.g., the NASA Enterprise Physical Access Control System for issuing and recording badges and other facility access credentials to employees, contractors, and visitors, IAW Homeland Security Presidential Directive 12 (HSPD-12). Obtain electronic fingerprints.
