About this file

This is a Notice of Intent to Award issued by the Oregon Department of Human Services (ODHS) and Oregon Health Authority (OHA) for Request for Proposals (RFP 6124), OregonBuys Solicitation Bid # S-10000-00013757. The RFP seeks qualified contractors to provide Agency with Choice (AwC) services, a self-directed service delivery model for older adults, individuals with physical disabilities, and individuals with behavioral health conditions. The state intended to award up to four contracts with a maximum of two successful proposers to provide comprehensive administrative support for individuals directing their own care services, including management of Direct Support Workers (DSWs) and facilitation of person-centered care planning. Proposals were due on August 25, 2025, at 9:00 AM PST. The initial contract term is anticipated to be two years with potential renewals up to a cumulative maximum of five years. The state has completed proposal evaluations and issued this Notice of Intent to Award on November 3, 2025, identifying Guardiantrac, LLC d/b/a GT Independence as the highest-ranked proposer, with contract negotiations to follow.

The fully loaded Medicaid reimbursable hourly rate for DSWs is $42.12 per hour, with a strict requirement that at least 85 percent of the total hourly rate be used for DSW compensation including wages, benefits, and insurance, capping the administrative fee at 15 percent. Additional compensation includes $0.70 per mile for mileage reimbursement and a 3-hour annual assessment fee per individual served. The state strongly encourages participation from diverse business enterprises, including small businesses, minority-owned businesses, woman-owned businesses, service-disabled veteran-owned businesses, and emerging small businesses certified by the Oregon Certification Office for Business Inclusion and Diversity (COBID). Successful contractors must obtain a Pay Equity Compliance Certificate for contracts exceeding $500,000 or involving 50 or more full-time workers and comply with House Bill 4129 labor relations requirements, including documentation of interactions with labor organizations representing Direct Support Workers. Contractors are subject to quarterly reporting, rigorous background checks, and strict financial transparency requirements.
