About this file
This document is a Proposer Information and Certification Sheet for an Oregon Request for Proposal (RFP) issued by the Oregon Department of Human Services (ODHS) and Oregon Health Authority (OHA), with the OregonBuys Bid Number S-10000-00013757. The document is Attachment F of the RFP and serves as a comprehensive certification form for potential bidders, requiring proposers to affirm compliance with various state and federal regulations, non-discrimination policies, and ethical standards.
The certification sheet covers multiple critical compliance areas, including tax law adherence, employment non-discrimination, wage equality, broadband service regulations, and conflict of interest disclosures. Proposers must certify they are not on federal exclusion lists, have independent proposal preparation practices, and understand potential legal consequences for false claims. The document also requires proposers to confirm they have (or will develop) written policies preventing sexual harassment and discrimination, and that they do not prohibit employees from discussing compensation. Notably, for contracts anticipated to be $150,000 or more, agencies will not enter into agreements with proposers lacking appropriate anti-discrimination policies.
