About this file
This is a Quality Assurance Surveillance Plan (QASP) that establishes performance monitoring procedures and standards for the Oily Wastewater (OWW) Treatment, Recycling, and Disposal Services contract in Sasebo, Japan. The QASP outlines a multi-functional team structure including an on-site Superintendent, Government Technical and Environmental Points of Contact (TPOC/EPOC), and a Contracting Officer, each with defined surveillance and oversight responsibilities. Performance evaluation occurs quarterly using a Contractor Performance Evaluation Sheet (CPES) completed by the Government EPOC, with an annual Contractor Performance Assessment Report (CPAR) submitted by the SRF TPOC.
The QASP establishes ten performance objectives with specific standards and Acceptable Quality Levels (AQLs) covering barge cleanliness, wastewater disposal compliance, license maintenance, environmental protection plan approval, environmental management system training, spill boom deployment, spill notification procedures, treatment process validation, industrial and hazardous waste disposal, and used oil recycling documentation. Contractor performance is rated as "Acceptable" or "Not Acceptable" based on whether AQLs are met, with evaluation methods including weekly no-notice barge inspections, 100% PFAS testing review before discharge, quarterly license and plan reviews, and 100% inspection of transfer operations and waste documentation. Non-conforming performance triggers mandatory corrective action with submission of a Corrective Action Plan within 30 calendar days, while consistently meeting AQLs serves as positive past performance documentation for future contract evaluations.

Per the FAR Subpart 42.15 and Navy's CPAR System Guide, CPAR is used for the annual evaluation of overall contract performance. The SRF TPOC evaluates based on CPES and other information. This report may affect future source selections to ensure that the Federal Government only does business with companies that provide quality products and services in support of the agency’s missions.
4. Record of Evaluation:
The Evaluators and Contracting Office maintain evaluation records.
| 5. | Incentive/Decrement: |
| a. | Incentives (Positive Performance) |
The Government will document favorable contractor performance via the Contractor Performance Assessment Reporting System (CPARS) and/or the local Contractor Performance Evaluation System (CPES). Consistently meeting or exceeding the Acceptable Quality Levels (AQLs) will serve as a primary incentive, as these positive past performance records will be utilized by the Government in the evaluation of future contract solicitations.
b. Disincentives (Non-Conforming Services) Disincentives for non-conforming OWW treatment and disposal services will be enforced in accordance with standard commercial contract remedies (e.g., FAR 52.212-4, Contract Terms and Conditions—Commercial Products and Commercial Services). The Government reserves the right to require the Contractor to re-perform any defective or non-conforming service to meet contract requirements at no additional cost to the Government.
c. Mandatory Corrective Action for Unacceptable Performance In the event the Contractor’s overall performance is rated 'Unacceptable' in a CPES and/or CPARS evaluation, the Contractor shall immediately initiate corrective action. The Contractor must rectify all identified deficiencies and submit a Corrective Action Plan (CAP) at no additional cost to the Government within thirty (30) calendar days of receiving the official evaluation report.
