About this file

This is a Past Performance Questionnaire template issued by the Department of Veterans Affairs (VA) Network Contracting Office 4 in Pittsburgh, Pennsylvania for use in evaluating contractor performance on the "Upgrade Medical Gas & Communication System" contract opportunity.

The form is designed to be completed by reference contacts who have directly worked with an offeror on previous projects similar to the current solicitation. Offerors must submit three completed questionnaires (one per past project) as part of their proposal experience section. Reference contacts are instructed to return completed forms directly to the contracting officers Donald Kalivoda (donald.kalivoda@va.gov) and Elijah McIntosh (elijah.mcintosh@va.gov). The questionnaire evaluates contractor performance across multiple categories using a 0-5 rating scale (Neutral to Outstanding): Quality—Management & Workmanship (6 items covering management experience, personnel qualification, quality control, deficiency correction, submittals, and problem-solving approaches); Timeliness and Adherence to Schedule (3 items addressing delay notices, submittal timeliness, and schedule realism); Business Practices/Customer Relationship and Ability to Perform (4 items covering independent performance, subcontractor management, and change order proposals); Compliance & Safety (4 items addressing reporting, federal law compliance, safety programs, and infection control); and Overall Customer Satisfaction (3 items on modification cost reasonableness, cure notices, and contractor re-award willingness). The form concludes with an overall performance rating and requires the reference contact to certify the accuracy of responses with signature, printed name, email, business title, and date.
