About this file

This is a Past Performance Questionnaire (PPQ) template for USFS Region 2 Road and Bridge Construction MATOC Solicitation 1240LU25R0008. The questionnaire is to be completed for any projects listed under Factor 1, Subfactor 1-Experience where CPARS evaluations are not available, and must be submitted to paul.larsen@usda.gov prior to the solicitation closing date.

The PPQ requires contractors to provide basic project information including contract value, description of work, duration, and completion status. The client then evaluates the contractor's performance across multiple categories using a 5-tier rating system (Outstanding to Unsatisfactory) in areas including quality, schedule compliance, client satisfaction, management effectiveness, cost control, and safety. The evaluation also includes a yes/no question about willingness to work with the contractor again and space for additional remarks. This PPQ is related to a small business set-aside MATOC for road and bridge construction services in USFS Region 2, with a base period of 2 years plus three 1-year options, and task orders ranging from $2,000 to $3,000,000 with a $50M shared ceiling.
