Past Performance Assessment Questionnaire Summary

This is a Past Performance Assessment Questionnaire template issued by the 87th Contracting Squadron to collect information about contractor performance on previously awarded contracts. The questionnaire concludes with a section requiring yes/no responses regarding contract reawarding, default termination, corrective action notices, and an overall performance rating selection, with mandatory comment fields for any Unsatisfactory, Marginal, or Exceptional ratings. Failure to provide supporting comments for extreme ratings renders the questionnaire void and unacceptable to the Government.
