PAST PERFORMANCE QUESTIONNAIRE (PPQ)

INSTRUCTIONS

The “Offeror” (identified in Section I, Block A) has identified you as a point of contact (POC) for a past and/or present performance evaluation to support their pending response to a solicitation issued by the Defense Finance and Accounting Service (DFAS) to provide banking operations service overseas for the DFAS Overseas Military Banking Program (OMBP).

DFAS is in the process of competitively selecting a source for the OMBP. One of the considerations in proposal evaluation is the verification of the Offeror’s past performance on contracts or other contract-like efforts which reflect the Offeror’s ability to perform on the proposed effort. We depend on organizations such as yours, which have had first-hand experience with the Offeror, for the evaluation of the Offeror’s performance on those contracts or contract-like instruments and business relationships.

Your candid responses to this questionnaire will assist DFAS in evaluating the Offeror’s past performance. (I.e., How was the quality of the services this Offeror provided in the past?) By gathering this information (not only on this Offeror, but on all the Contractors [Offerors] who submit an offer to provide the required services) the Government strives to determine which offer will be most advantageous to the Department of Defense (DoD).

We appreciate the time and effort you take to complete Section III and Section IV (if applicable) regarding the services you received from the Offeror.

CODE PERFORMANCE LEVEL

5 EXCEPTIONAL - Performance meets contractual requirements and exceeds many to the Government’s/organization’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.

4 VERY GOOD - Performance meets contractual requirements and exceeds some to the Government’s/organization’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the Offeror were effective.

3 SATISFACTORY - Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the Offeror appear or were satisfactory.

2 MARGINAL - Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the Offeror has not yet identified corrective actions. The Offeror’s proposed actions appear only marginally effective or were not fully implemented.

1 UNSATISFACTORY - Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element being contains serious problem(s) for which the Offeror’s corrective actions appear or were ineffective.
