(b) The offeror shall submit the following information:
(1) Responsibility Determination Binder: 2 (1 original and 1 copy) copies of the responsibility determination information. With the responsibility determination binder, submit the following:
a. Submit completed and signed Standard Form 33 with the applicable Entity Identifier number and CAGE code of the offeror provided on the face of the SF-33. Ensure the SF-33 includes the appropriate name and address of any newly formed joint venture/partnership arrangement if applicable;
b. Submit acknowledgement of all amendments;
c. Identify the company names, CAGE Code, and Entity Identifier for all team members and identify their role (prime, joint venture partner, significant subcontractor, subcontractor, etc.);
d. Submit individual representations and certifications if System for Award Management (SAM) representations and certifications are not complete in accordance with FAR 52.204-7;
e. Provide confirmation of most recent VETS-4212 filing or an explanation of the offeror’s exemption from this requirement;
f. The offeror shall provide the latest three complete fiscal year financial statements, certified by an independent accounting firm or at least by an authorized officer of the organization. If the offeror is a new joint venture or partnership, include the latest three complete fiscal year financial statements for each firm, certified by an independent accounting firm or at least by an authorized officer of the organization.
g. Submit evidence of availability of working/operating capital from an independent financial institution, which will be used for the performance of the resultant contract.
h. If joint venture or partnership arrangement is being proposed, include copies of agreements and letters of commitment signed by the appropriate individuals of each firm. Agreements must include company name, Entity Identifier number, address, point of contact, email address, phone number and fax number for the joint venture/partnership and each member of the joint venture/partnership. The agreements shall demonstrate the relationship between firms and identify contractual relationships and authorities to bind the firm/joint venture/team/partnership. Offerors proposing significant subcontracts (subcontracts exceeding 10% of the total contract value) shall submit signed letters of commitment from the subcontractors which define the contractual relationship and identify contribution to the planned arrangement in terms of the type and proportion of work to be performed. For Joint Venture arrangements discuss the financial responsibilities among the companies.
i. Offerors shall submit plan in accordance with DFARS 252.237-7024 NOTICE OF
CONTINUATION OF ESSENTIAL CONTRACTOR SERVICES (OCT 2010).
j. Submit two copies of the entire Responsibility Determination information on CD filed inside the cover of the "Original" hard copy Responsibility Determination binder. Note: do NOT submit a USB flash/jump drive in lieu of CD.
k. Paper should be white, 8 ½ by 11 inches and have margins of 1 inch on all four sides. All text (including text in all graphics, exhibits, tables, and headers/footers) will be in a font no smaller than 11 point, Times New Roman or Courier. Colored font may be used for headers/footers, section headings and subheadings only; all other text will be black. Double-side printing is permitted;
l. There is no page limitation on the responsibility determination submission.
(2) Price Proposal Binder: 4 (1 original and 3 copies) copies of the price proposal. With the price proposal, submit the following:

Offerors shall utilize the ELIN pricing attachment provided in Section J to submit the ELIN pricing data in the format provided. Formulas should be traceable. Failure to do so may be cause to render the offer non-responsive.
Because the pricing evaluation will concentrate on the electronic files submitted, in the event of a discrepancy between the price proposal hard copies and the price proposal electronic copies, the price proposal electronic copies will take precedence.
f. Paper should be white, 8 ½ by 11 inches and have margins of 1 inch on all four sides. All text (including text in all graphics, exhibits, tables, and headers/footers) will be in a font no smaller than 11 point, Times New Roman or Courier, with single line spacing. NOTE:

Colored font may be used for headers/footers, section headings, and subheadings only; all other text will be black. Double-side printing is permitted; however, each side will be counted as one page for sections with page limitations. For larger spreadsheets, the offeror may submit 11 by 17 inch sheets that must be folded to fit within the binder, with printing on one side only;
g. Offerors shall put company name on every pricing page.
h. With the exception of the price narrative, there is no page limitation on the pricing proposal.
(3) Non-Price Proposal Binder: 6 (1 original and 5 copies) copies of the non-price proposal. With the non-price proposal, submit the following:

c. Submit two copies of the entire Non-Price Proposal on CD in readable/searchable electronic format and filed inside the cover of the "Original" hard copy Non-Price Proposal binder.
Acceptable readable/searchable formats include Word, Excel, and searchable PDF. Note: do NOT submit a USB flash/jump drive in lieu of CD. For the JL-1 Attachment, ensure data is submitted in hard copy and in Excel (Microsoft Office Version 2016 compatible) format on the above mentioned CD. In the event of a discrepancy between the JL-1 Attachment hard copies and the JL-1 Attachment electronic copies, the JL-1 Attachment electronic copies will take precedence. For the remainder of the Non-Price Proposal, in the event of a discrepancy between the hard copies and the electronic copies, the Non-Price proposal hard copies will take precedence.
d. State a clear acknowledgement within the beginning of the binder that nothing in the following proposal is meant to supersede or reduce any of the individual requirements to which the offer addresses. It is the Government’s intent to incorporate the offeror’s proposal into the final contract while at the same time eliminating any contradictory or conflicting requirements between the two. In the event of a conflict between any part or parts of the offer and part or parts of the requirement, the Government has the right to selectively choose that part or those parts which are the more advantageous of the two.
e. Paper should be white, 8 ½ by 11 inches and have margins of 1 inch on all four sides. NOTE:
Margins in forms and attachments provided by the Government are exempt from the margin size requirement and shall be submitted in the format provided. All text (including text in all graphics, exhibits, tables, and headers/footers) will be in a font no smaller than 11 point, Times New Roman or Courier, with single line spacing. Colored font may be used for headers/footers, section headings, and subheadings only; all other text will be black. Double-side printing is permitted; however, each side will be counted as one page.
f. Offerors shall put company name on every Non-Price page.
g. The non-price proposal submittal shall not exceed 150 pages. Blank tab dividers, Past
Performance Questionnaires, and Contractor Performance Assessment Reports will not count toward the page limitation.
(4) All binder covers and spine inserts will contain the solicitation number and title, the offeror’s name including individual point of contact, with phone number AND email address. It is acceptable to include the individual point of contact, with phone number and email adress on the cover only and not on the spine.
(5) Offerors are cautioned that “parroting” of the Performance Work Statement Performance
Objectives with a statement of intent to perform does not reflect an understanding of the requirement or capability to perform.
SECTION M - EVALUATION FACTORS FOR AWARD
M.3 EVALUATION FACTORS
1. The solicitation requires the evaluation of price and the following non-cost/price factors:
Factor 1 – Corporate Experience Factor 2 – Technical Approach Factor 3 – Safety Factor 4 – Small Business Utilization Factor 5 – Past Performance
2. The relative order of importance of the non-cost/price evaluation factors is the technical Factors 1, 2, 3, and 4 are of equal importance to each other and, when combined, are equal in importance to the past performance evaluation/performance confidence assessment Factor 5. When the proposal is evaluated as a whole, the technical factors and past performance/performance confidence assessment factor combined (i.e., the non-cost/price evaluation factors) are significantly more important than price.
3. Basis of Evaluation and Submittal Requirements for Each Factor.
(a) Price:
(1) Solicitation Submittal Requirements:
(i) Provide a completed Section B of the solicitation.
(ii) Provide a completed Section J Exhibit Line Item Number (ELIN) Pricing Attachment.
There are multiple worksheets within this file that require completion. Proposed prices for Spec Item 3 Labor Cost, Spec Item 3 Material, Spec Item 3 Equipment, Spec Items 1 and 2 On-Site Cost, Off-Site Cost, and Profit/Fee shall be provided in the same level of detail for all requirements, whether the work is proposed to be accomplished as the prime contractor, subcontractor, or by other means.
(iii) Submit pricing data (Section B and Section J ELINs) in hard copy and in electronic
(Microsoft Office Version 2016 compatible) format on CD-ROM. Offerors shall utilize the pricing attachment provided in Section J to submit the ELIN pricing data in the format provided. Because the pricing evaluation will concentrate on the electronic files submitted, in the event of a discrepancy between the proposal hard copies and the proposal electronic copies, the proposal electronic copies will take precedence.
(iv) Provide a narrative (limited to 15 pages; double-sided printing is allowed; however, each side will be counted as one page) of all costs your firm classifies as direct and indirect and describe the basis and rationale for the proposed amounts. Explain how these costs are allocated to each of the cost elements identified in the Section J ELIN Attachment (Spec Item 3 Labor, Spec Item 3 Material, Spec Item 3 Equipment, Off-Site Cost, etc.).
(2) Basis of Evaluation:
(i) The Government will evaluate price based on the total proposed price, taking into consideration proposed ELIN Pricing at the functional (Annex/Sub-annex, Spec Item, etc.) and elemental (Spec Item 3 Labor, Spec Item 3 Material, Spec Item 3 Equipment, etc.) level. Total price consists of the basic requirements and all option items (see Section B of the solicitation). The Government intends to evaluate all options and has included the provision FAR 52.217-5, Evaluation of Options (JUL 1990) in Section M of the solicitation. In accordance with FAR 52.217-5, evaluation of options will not obligate the Government to exercise the option(s). Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:
(a) Comparison of proposed prices received in response to the RFP.
(b) Comparison of proposed prices with the IGE.
(c) Comparison of proposed prices with available historical information.
(d) Comparison of market survey results.
(e) And/or any other methodology deemed appropriate by the Contracting Officer and consistent with FAR 15.404-1.
(ii) Identification of unbalanced pricing including allocation to the ELIN/Sub-ELIN unit prices. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more prices at the functional (Annex/Sub-annex, Spec Item, etc.) level, ELIN/sub-ELIN unit prices, or contract line items is significantly overstated or understated. An offer may be rejected if the proposed price is materially unbalanced between line items or sub-line items within a pricing period, or between line items or sub-line items for the basic requirement and any option period.
(iii) Any suspected issues or concerns in the price proposal, including any inconsistent pricing submitted within Section B and the Section J ELIN attachment.
(iv) Any inconsistency, whether real or apparent, between proposed performance and price must be clearly explained in the price proposal. For example, if unique and innovative approaches are the basis for an apparently unbalanced/inconsistently priced proposal, the nature of these approaches and their impact on price must be completely documented. It is the responsibility of the offeror to provide all required documentation.
(b) Non-cost/price Factors:
(1) Factor 1 – Corporate Experience
(i) Solicitation Submittal Requirements: The offeror shall provide a minimum of two (2) and a maximum of five (5) examples of relevant projects to be documented only on Attachment JM-1, Corporate Experience Project Data Sheet. Offerors shall not submit any information under Factor 1 unless submitted on Attachment JM-1. Information submitted for a corporate affiliate to include a one page narrative, organization chart and teaming agreement/letter of commitment may be provided in addition to the Attachment JM-1. The offeror shall clearly demonstrate recent, relevant experience as a prime contractor on projects similar in size, scope, and complexity to the requirements described in the Performance Work Statement (PWS) of the RFP.
RELEVANT PROJECT: Relevant projects must demonstrate ALL of the following:
a. The capability to perform services similar in size, scope, and complexity to those described in the performance work statement/specifications of the RFP,
b. a contract value of $12,000,000.00 per year or greater; and,
c. shall be: 1) ongoing with at least 12 months of contract performance completed by the date of the solicitation issuance, OR 2) shall have been completed within the last five (5) years.
For purposes of this evaluation, a relevant project is further defined as Base Operations Support (BOS) services (or equivalent) that encompass at least two of the following:
a. 1501000/1502000 – Facility Management/Facility Investment (including at least 3 sub-categories, such as Infrastructure Condition Assessment Programs, Scoping and Estimating Services, Managing Service Orders, Preventive Maintenance Programs, Integrated Maintenance Programs, Inspecting, Testing and Certification Programs or Work Order Management).
b. At least two Utility sub-annexes that are similar to the respective requirements in the RFP (at minimum, including operations, maintenance and repairs of the sub-annex systems.).
c. 1700000 – Base Support Vehicles and Equipment (BSVE) (at minimum, including operations, maintenance and repair of Material Handling Equipment (MHE) and Civil Engineering Support Equipment (CESE) similar to the requirements of this RFP).
The offeror’s submitted projects must aggregately demonstrate experience self-performing or managing ALL of the following:
a. 1501000/1502000 – Facility Management/Facility Investment (including at least 3 sub-categories, such as Infrastructure Condition Assessment Programs, Scoping and Estimating Services, Managing Service Orders, Preventive Maintenance Programs, Integrated Maintenance Programs, Inspecting, Testing and Certification Programs).
b. At least two Utility sub-annexes that are similar to the respective requirements in the RFP (at minimum, including operations, maintenance and repairs of the sub-annex systems.)
c. 1700000 – Base Support Vehicles and Equipment (BSVE) (at minimum, including operations, maintenance and repair of Material Handling Equipment (MHE) and Civil Engineering Support Equipment (CESE) similar to the requirements of this RFP).
If the offeror is a Joint Venture (JV), recent/relevant experience should be submitted for projects completed by the JV entity. If the JV does not have such experience, recent/relevant experience shall be submitted for each JV partner (except for offerors covered by Mentor-Protégé Agreements approved by the Small Business Administration). JV offerors are still limited to a total of five (5) recent and relevant projects.
