About this file

This document is an Electrical Manhole Inspections Report Requirements and Sample Report for Manhole #19, part of a project to replace oil switches for the Naval Facilities Command (NAVFAC) Mid-Atlantic region. The inspection requirements include a comprehensive survey of an existing electrical manhole, with detailed specifications for documenting its condition, cable layouts, and infrastructure.

Key inspection requirements include: performing a field investigation to document the manhole's condition, recording dimensions and identifying all cables and circuits, checking duct and circuit integrity, and providing detailed photographic evidence. The contractor must have qualified personnel with safety training, prepare safety documents, remove water if present, and notify NAVFAC MIDLANT UEM at least 48 hours before inspection. The Electrical Manhole Inspection Report (EMIR) must be completed within 15 days of the kick-off meeting and include comprehensive details such as manhole size, conduit locations, cable information, circuit identifications, and any safety issues discovered during the inspection.

Electrical Manhole Inspections Report Requirements and Sample Report

This project will survey existing EMH #467.

1. Perform field investigation and document existing condition of EMH#467. Check the integrity of existing ducts and circuits, identify all existing cables, circuit routing and provide details of cable layouts found within the manholes inspected.

2. Record dimensions of all sides and depth of the manhole. Report any defects discovered, including materials, obstructions, breakage, connections and deterioration. The visual inspections shall include all interior surfaces, cables, manhole cover and frame, chimney, ladders, racks, walls, invert and all appurtenances.

Provide photos of each side and all defects. Inspections may be performed by personnel entry of from the surface utilizing pole mounted digital camera equipment, provided the detail is sufficiently thorough.

3. The electrical inspections contractor must have an internal quality assurance/quality control program in place.

Qualified personnel performing the inspections shall demonstrate by training and experience the ability to safely enter underground structures, work on or near exposed energized equipment or conductors, recognize types of cables and splices, abnormal conditions and perform manhole inspections. Qualified personnel shall be trained in and familiar with the clearance distances and corresponding voltages to which they will be exposed and confined space training.

4. Prepare safety documents including a safety plan, confined space permits and Activity Hazard Analysis

(AHA).

5. Existing manhole may be filled with water. The contractor shall remove all water by pumping as required for safe entry.

6. The contractor is responsible for notifying NAVFAC MIDLANT UEM a minimum of 48 hours prior to performing any inspection work.

7. The contractor shall notify NAVFAC MIDLANT UEM immediately if any defects found pose an imminent danger to the public or the integrity of the utility system (i.e., damaged or shorted cables, missing or broken lids, indications of collapse, etc.).

8. The electrical manhole inspection and report documents shall be completed within 15 days after kick-off meeting.

9. Electrical Manhole Inspection Report (EMIR) of inspection results, including the manhole layouts shall include, but not limited to the following data:

a. Provide manhole number and detailed location.

b. Indicate condition of manhole. Record if manhole was flooded and pumped dry to accomplish inspections. If significant debris is present, record the type and depth of the debris and if removal was required.

c. Indicate the manhole size (length, width and height).

d. Indicate size of manhole opening (lid size, depth of throat, opening size), cover and frame condition and rim to grade distance (inches).

e. Indicate true north arrow.

f. Provide detailed layout of each side of the manhole detailing conduit locations and size of conduit openings.

g. Provide cable information, i.e., circuit identification, splices, dead end/capped cables, abandoned, and damaged insulation.

h. Provide a sheet for each size of the manhole to include:

1) Number of ductbanks in side. If more than one, label and provide separate sheet for each.

2) Number and size of used conduits and conduit material.

3) Number and size of spare conduits and conduit material. Note if pull ropes are present.

4) Identify collapsed ducts.

5) Distance from floor to conduits.

6) Distance from left wall to conduits.

7) Distance from right wall to conduits.

8) Circuit(s) identification.

9) Layout of side.

10) Identify ductbanks and their connecting entity, i.e., manhole #, switch #, transformer, substation, etc. This information is not always evident from looking at the tags. This will help situations where the tag information is vague or when two or more ductbanks leave the same side of a manhole for different destinations.

i. Provide photograph of each side of manhole.

j. Detail any safety issues in manhole as follows:

k. Racks deteriorated or missing.

l. Grounds, verify all cable shields are grounded and identity where cable grounds are missing.

m. Integrity of manhole, cover and frame, etc.

n. Damaged conductor insulation.

o. Indicate approximate water level upon opening of manhole.

p. Complete the Confined Space Entry Permit and identify if hazardous gas was present upon opening of lid.

q. Provide inspection personnel names and point of contact information.

r. Provide a new tag in manhole to require personnel entering manhole to contact NAVFAC MIDLANT

UEM within 24 hours of their presence within manhole if not notified previously and to identify any changes made to manhole layout under normal or emergency conditions.

s. Indicate if ground rods are present in manhole.
