*Small Business (SB) includes Small Businesses, Small Disadvantaged Businesses, Women-Owned Small Businesses, Veteran-Owned Small Businesses, Service-Disabled Veteran-Owned Small Businesses, HUBZone Small Businesses, and Historically Black Colleges and Universities and Minority Institutions.
1. Specifically identify the SB* firms with whom you, as the offeror, will perform work under this planned contract and to what extent (percentage) these firms will participate in the total acquisition value. (Note: SB* primes are to include their own effort)

2. Describe the tasks each Small Business subcontractor will perform and how the tasks assigned to Small Businesses are meaningful to the overall success of the program.
3. If, historically, the offeror did not meet or exceed small business goals, the offeror shall provide a detailed explanation addressing why goals were not met what actions it took to resolve previous failures and what actions will be taken to meet the goals on this effort.
