Q1 Is it possible to get a copy of the Fermilab Subcontractor Injury & Illness Data Questionnaire in Word format?

A1 A Microsoft Word version of the Fermilab Subcontractor Injury and Illness

Questionnaire is attached.

Q2 Can the requirements for the 4-gas meter be confirmed? Specifically, that CO2

(Carbon Dioxide) is required, not CO (Carbon Monoxide).

A2 The SOW states “Gas monitors shall have capacity to measure NO2, O2, CO2, H2S”. The current version of the SOW is not correct. The requirement is “NO2, O2, CO, H2S”.

Q3 There is a listed requirement for proximity sensors on hard hats. There is not a listed requirement for lights and hearing protection on hard hats. Are lights and hearing protection required for hard hats? Also, are there any other PPE requirements outside of standard Level D?

A3

• SOW Section 4.5 states “Type II Safety Helmet with cap lamp and chin strap” o Yes--a light mounted on the type II safety helmet is required for underground access o Hearing protection is also required based on the task--it is not required to be mounted to the type II safety helmet but provided as required for the task. If double hearing protection is required, then logic would prevail that safety helmet mounted ear muffs would be required to accomplish the required level of protection beyond ear plugs

• Additional requirements that exceed ‘level D’ include the listed PPE of “Reflective clothing (Class II)” and “Level IV cut resistant gloves”, both listed in the SOW
