About this file
This is a Past Performance Questionnaire for solicitation FA301026R0010 issued by the 81st Contracting Squadron at Keesler Air Force Base, Mississippi, for renovating Stennis Hall. The questionnaire must be completed and submitted by email to alicia.bowman@us.af.mil no later than 09:00 AM CDT on 27 August 2026. Responses must be submitted directly to the Contracting Officer from a corporate or government email account. The form requests identification of an alternate point of contact if the primary contact is unavailable on the due date.
The questionnaire requests references to evaluate a contractor's past performance across five key categories: Quality of Work, Schedule (including timeliness and ability to resolve timeliness problems), Management (ability to manage all aspects of work), Regulatory Compliance (including safety regulations, Davis-Bacon Act, and other labor standards), and Overall Performance Rating. Evaluators must rate performance using six standardized levels: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, and Not Applicable. The form includes detailed evaluation definitions for each rating level and requires respondents to provide the referenced contract number, contract value including modifications, period of performance, and a brief description of work required. Explanatory narratives with specific details are requested for each evaluation section, with the option to attach additional pages as needed.
