Past Performance Questionnaire

PAST PERFORMANCE QUESTIONNAIRE

CONSOLIDATED OPERATIONS, MANAGEMENT, ENGINEERING AND TEST (COMET) CONTRACT

John F. Kennedy Space Center, Florida

This questionnaire is submitted for your evaluation by a company that has intentions to compete for the Consolidated Operations, Management, Engineering and Test (COMET) contract at the Kennedy Space Center as a prime or subcontractor. The following terms, “customer reference” and “evaluator,” are used to refer to the person performing the evaluation. For the COMET Request for Proposal (RFP), NASA requires the company to provide this questionnaire to customer references for which it has had an active or completed contract in the past five years. Please complete this questionnaire and return to NASA per the following instructions. Your assistance in this effort is greatly appreciated.

The purpose of the COMET contract is to enable the evolution of exploration mission services and the principal functions are to provide safe operations, maintenance and processing of complex launch-related ground systems that require skilled engineering and integrated management. This contract, Consolidated Operations Management Engineering and Test (COMET) will succeed KSC’s Test and Operations Support Contract or TOSC. COMET will provide engineering; ground system development; flight vehicle/spacecraft processing; and launch, landing, and recovery operations for the following Exploration Systems Development (ESD) programs: Exploration Ground Systems (EGS), Space Launch System (SLS), and Orion Multi-Purpose Crew Vehicle (MPCV) Programs. This contract will provide services to International Space Station (ISS), Launch Services Program (LSP) as well as other NASA programs, commercial ventures, and partnerships at KSC.

Instructions for completing the Past Performance questionnaire and a sample cover letter to send to your customer reference for performance assessment are included in the following pages.

Please request that the customer reference complete the attached questionnaire and email a PDF file, to Christina Johnsen, Contracting Officer, at . The completed questionnaire will become a part of the official source selection records. To be appropriately considered, the Past Performance Questionnaire shall be returned to the Contracting Officer in its entirety no later than: TBD. The National Aeronautics and Space Administration (NASA) Kennedy Space Center (KSC) is in the process of soliciting offers from companies for the Consolidated Operations, Management, Engineering and Test (COMET) contract at the Kennedy Space Center. ___________________ has provided your name and organization as a reference regarding our record of past performance under Contract Number: .

Your completed questionnaire will become a part of the official source selection records. To be appropriately considered, the Past Performance Questionnaire shall be returned to the Contracting Officer in its entirety no later 8:00 AM EDT, on April 18, 2022.

QUESTIONNAIRE INSTRUCTIONS

Sections I and II - Company and Contract Information: To be completed by the Offeror.

Section III - Evaluator Information: To be completed by the evaluator. The COMET Contract Source Evaluation Board may contact you regarding the information you provide.

Section IV: Performance Survey: To be completed by the evaluator. Provide your assessment of the company’s experience level and performance, as evidenced within the contract for which you are a customer reference. It is very important to keep in mind that only performance on active or completed contracts in the last five years is relevant.

For each item, respond by placing an “x” in the appropriate block for both the Experience Level and your Performance Assessment that best describes the company for the referenced contract or order.

Please use the following definitions in assessing the company’s experience level.

Significant (SIG)
Full range of services was routinely performed by the contractor for the applicable element
Moderate (MOD)
Experience in several aspects of an element, even though the experience may not have been on a continuous basis or directly related to the purpose of COMET.
Minimal / Not Perform (Min/NP)
At least some aspects of the element may have been performed, such performance was limited in scope or frequency; or the element was not performed under the contract.

Please use the following performance ratings for items the company has significant or moderate experience.

Exceptional (E)
The company’s past performance is of exceptional merit; exemplary performance in a timely, efficient, and economical manner; very minor (if any) problems with no adverse effect on overall performance.
Very Good (VG)
The company’s past performance is very effective; fully responsive to contract requirements accomplished in a timely, efficient, and economical manner for the most part with only minor problems with little identifiable effect on overall performance.
Satisfactory (S)
The company’s past performance is effective; fully responsive to contract requirements; reportable problems, but with little identifiable effect on overall performance.
Marginal (M)
The company’s past performance meets or slightly exceeds minimum acceptable standards; adequate results; reportable problems with identifiable, but not substantial, effects on overall performance.
Unsatisfactory (U)
The company’s past performance does not meet minimum acceptable standards in one or more areas; remedial action required in one or more areas; problems in one or more areas which adversely affect overall performance.
