About this file
This is a Cost Proposal Form for the Kentucky Transportation Cabinet's (KYTC) Electric Vehicle Charging Program RFP 4. The form serves as the cost submission template for proposers seeking reimbursement for electric vehicle supply equipment (EVSE) infrastructure projects. Proposers must complete designated yellow cells with capital costs, operations and maintenance (O&M) costs across five years, and requested reimbursement percentages, while the white cells auto-populate calculations based on these inputs. Capital cost categories include pre-construction costs, construction costs, EV charger acquisition costs, utility upgrades, workforce development activities, and miscellaneous capital costs such as ADA compliance upgrades, commissioning, and pre-purchased warranties. O&M costs span five years and include warranty costs, site maintenance, internet and network fees, software fees, EVSE maintenance and repair, data sharing and reporting, cybersecurity, electricity costs, insurance, customer service, workforce development, and administrative costs.
The cost proposal establishes strict parameters for reimbursement eligibility and cost allocation. All proposers are eligible to receive up to $150,000 in maximum reimbursement for demand charges over the project life, though demand charges must not be included in the cost proposal itself. Proposers must maintain the same O&M requested reimbursement percentage across all five years, and actual costs may shift between categories except for demand charges, which cannot be reallocated. Critical restrictions prohibit proposers from submitting warranty costs in both capital and O&M categories simultaneously and similarly restrict EVSE costs to one category only; for shared NEVI and non-NEVI ports, only costs proportional to NEVI port power rating are eligible. The Maximum Total Project Reimbursement is subject to KYTC confirmation that all proposed costs comply with 23 CFR 680 eligibility requirements.

| DO NOT adjust any of the formulas or fill information in the white boxes. The white boxes will auto populate based upon inputs from yellow boxes. | |
| If awarded, Awardees will be held to the Total Capital Costs, Total O&M Costs, and the Maximum Total Project Reimbursement (and percentages). Actual costs can shift within those categories, EXCEPT for Demand Charges. Unused amounts for Demand Charges CANNOT shift to other categories. | |
| * O&M Requested Reimbursement Percentage (%) must be the same for all 5 years | |
| **The Maximum Total Project Reimbursement is subject to confirmation by KYTC that all proposed costs outlined in this proposal are eligible per 23 CFR 680. | |
| *** Proposers can only submit warranty costs in Capital Costs OR O&M Costs, but NOT BOTH. | |
| **** Proposers can only submit EVSE costs in Capital Costs OR O&M Costs, but NOT BOTH. If the EVSE is shared between NEVI and non-NEVI ports, Proposers can only include costs proportional to the power rating of the equipment that can supply the NEVI port simultaneously. | |
| All Proposers are eligible to receive up to $150,000 in reimbursement for demand charges over the life of the project to each awarded project. |
