About this file
This is a Required Affidavit for Bidders, Offerors, and Contractors mandated by the Commonwealth of Kentucky for the Electric Vehicle Charging Program RFP 4. The affidavit, governed by Kentucky Revised Statutes 45A.110 and 45A.115, must be completed and notarized by an authorized representative of any bidder or offeror seeking to be awarded a contract or contract renewal. The affidavit is valid for one year from the date of execution and requires the contractor to attest under penalty of perjury that it has not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any provision of those laws.
The affidavit requires the authorized representative's signature, printed name, title, and date, along with the bidder or offeror's name, address, and Commonwealth of Kentucky vendor code if known. The document must be subscribed and sworn before a notary public, with the notary providing their name, the date, state, county, and commission expiration date. The term "knowingly" is defined to include situations where the bidder or offeror is aware or should have been aware of a violation. The Commonwealth retains the right to request an updated affidavit at any time throughout the contract period.
