Section 1 – Purpose/Objective This Quality Control Program (QCP) takes all aspects of Quality Control into consideration to include but not limited to: quality of work, timeliness, positive appearance and attitude of employees, contract compliance, employee training, company safety program, and customer/tenant satisfaction. This plan will be updated annually and/or when changes are made. Quality of Services: Cleaning has a purpose and the five major reasons for cleaning are:
A. Prevention of the spread of infection and disease.
B. Dust control.
C. Preservation of the fabric, fixture, fittings and furnishings.
D. Provision of a socially acceptable environment.
E. Safety.
Quality control takes place by inspecting:
A. Work performed and quality. How well it was carried out?
B. Necessary activities and quality. How well they were carried out?
C. Cleaning frequencies and quality. How they are met?
D. Methods applied and quality. Are they the best practices?
Section 2 – Roles and Responsibilities of Key Personnel The following @@Company name officials will participate in assessing the quality of our performance. Their roles and responsibilities are described as follows:
2.1 Contract Manager (CM) @@Name/Contact information – The role and responsibility of the CM in support of the QCP:
1. Well versed and proficient with respect to contract specifications, terms, and conditions
2. Authorized to negotiate, administer the contract and take actions on behalf of @@Company name.
3. Ensure that quality control procedures set forth in the Quality Control Plan are consistent with the contract requirements.
4. Formulate, administer and modify the QCP as needed to deal with new or site-specific problems.
5. Initiate and maintain lines of communication on quality control policies, procedures and actual performance with the COR.
6. Act in an advisory capacity on all quality control matters.
7. Conduct quarterly QC self-evaluations and submit reports to the COR.
8. Conduct semi-annual quality control inspections of all custodial activities to ensure compliance with contract specifications/frequencies.
9. Maintain quality control program files, forms and records.
10. Recommend and monitor corrective actions.
11. Participate in partnering meetings.
2.2 Project Manager (PM) (On-site Supervisor) @@Name/Contact information - The role and responsibility of the PM in support of the QCP:
1. Well versed and proficient with respect to contract specifications, terms, and conditions.
2. Perform/ensure all custodial activities are performed in accordance with the approved schedules, at acceptable quality levels, in compliance with contract specifications and QCP standards.
3. Perform/ensure/recommend corrective actions for deficiencies identified during inspections.
4. Impart a positive work ethic and an appreciation for quality workmanship to employees.
5. Keep the CM apprised of potential QC problems and other matters pertaining to quality performance.
6. Participate in partnering meetings
2.3 Contract Employee – The role and responsibility of the Contract Employee in support of the QCP:
1. Deliver services only in accordance with approved safety practices, work methods, and procedures.
2. Report improper or unsafe conditions and practices immediately to the PM/On-site Supervisor
3. Undertake no job that is not fully understood or they are not qualified to perform.
4. Make suggestions to management for the improvement of quality and quantity of cleaning services provided.
5. Attend partnering meeting as directed.
2.4 Subcontractors – The same quality control procedures that govern performance also apply to all subcontracted services. These subcontracts are for @@window washing, snow removal, pest control and landscape maintenance. Subcontractors are selected by the following methods:
1. The supplier is capable of meeting the requirements of the contract.
2. Past work performance.
3. Best value
Each time a service is provided by the subcontractor, the service will be evaluated. @@Company name will look at on-time delivery, meeting specifications, and quality and competitive pricing. @@Company name will inspect 100% of the subcontractor’s work and the subcontractor’s performance will be evaluated. Subcontractors must receive a security clearance in order to be unescorted in the building. Subcontractors, without clearances, will remain in the company of @@Company name personnel at all times in which they are on premises. The list of subcontractors can be found below:
2.5 Staffing Level (including supervision)/ Work Schedules – The staffing levels depicting various job classifications (including sub-contractors) is below in our operating personnel chart.

EMPLOYEE WORK SCHEDULE
2.6 Roster – The following is an employee roster by name and job title along with job description.
Section 3 – Partnering In order to have an effective contract that also ensures a successful business relationship between GSA and @@Company name, all parties involved in the service delivery process must work as a team, foster open and honest communication at all times. Close coordination and active cooperation on a continuous basis between @@Company name and those representing the Contracting Officer are necessary to ensure a clean, safe, healthy work environment, and a well operated and maintained building. An initial step in avoiding disputes and claims is to settle minor problems and misunderstandings at the lowest possible level. Accordingly, @@Company name representative will meet with the COR or their representative and have meetings as needed to resolve contract issues. Partnering meetings will be as often as necessary at the discretion of the COR. The COR will meet as needed with the CM/PM to review service calls and inspection reports. A mutual effort will be made to resolve all problems identified during these meetings.
Section 4 – Effectiveness of Quality Control Plan
4.1 Quality and Quantity of supplies - @@Company name plans to use high quality supplies which we have found reduces waste. @@Company name quantity approach is to establish daily usage then carry out a risk analysis to determine how much stock should be maintained to cover any potential interruption in supply. @@Company name will order supplies based on a schedule so that adequate supplies should always be available on-site.
4.2 Task and Frequencies - This is a performance based contract. General custodial services will be provided Monday thru Friday, excluding federal holidays. Weekends may be utilized to complete additional obligations in the contract if necessary with COR approval. The cleaning schedule and work schedule can be found in Exhibit #1. Our QCP separates cleaning procedures by function and includes daily, weekly, monthly and annual objectives for various areas. Our QCP assigns high and low priorities to these areas. Areas may include the following space designations: entrances, lobbies and corridors; offices, courtrooms, conference rooms, restrooms/bathrooms, cafeterias and kitchens. Daily cleaning duties entail vacuuming carpets and mats, removing soil spots, emptying trash receptacles, cleaning restrooms, cleaning floors, refilling restroom supplies, cleaning entrance door glass panels and policing the grounds. Monthly duties may include dusting, spot cleaning walls or restoring the finish on resilient floors.
4.2.1 Trash and Recycling - All trash (including trash in restrooms) will be collected and removed to the location designated by the COR. Trash containers will be emptied and kept clean, odor-free and free of dirt, stains, dust, debris, residue and lack spillage. Plastic liners for all trash, debris and recycling containers will not be torn, worn, or contain residue. Ash receptacles will be free of dust, ashes, odors, tar, streaks and tobacco stains. @@Company name will provide the labor, equipment and means to collect and transport recyclable materials from centralized recycling bins located throughout the building to the collection/storage area. The Government provides the centralized recycling containers and desk side containers for the tenants for the collection of recycling. The building tenant is responsible for emptying their desk side container into centralized containers. Centralized containers will be emptied as needed. It is the responsibility of the Government to replace the desk side and centralized containers as needed upon notification from @@Company name. Paper trash in the mailroom shall be disposed of and NOT recycled.
4.2.2 Floors - Carpeted Areas and Walk Off Rugs - Carpets will be free of dirt and debris. Carpets will be free of spills, build up, obvious spots and smears and free of crusted material. Vacuuming will be done at a frequency that will protect the integrity of the carpet to prolong wear. Spot cleaning will be completed as needed to remove soiled traffic patterns, dirt, dust, debris, gum and crusted material. The walk off mats and runners and will be free of removable spots, dirt, dust, debris, soiled traffic patterns, gum and crusted material. The walk off mats will be cleaned by extraction annually and as needed or requested by the COR in accordance to the manufacturer’s recommended care instructions. The runners and walk off mats are furnished by the government. Walk off mats will be cleaned and inspected regularly. Extra runners and walk off mats, which may be used for inclement weather, will be cleaned and inspected by both the Project Manager and COR prior to storing.
4.2.3 Interior Hard Floor Surfaces - All floors will be maintained according to best trade practices. All bare floors and base moldings will be clean and free of dirt, dust and debris. Floors will maintain their natural luster and not have a dull appearance. Recommended cleaning mop and chemical detergent depending on the type of the floor will be used. There will be no buildup in corners or along walls. No water solution will be used on wood flooring.
4.2.4 Restrooms and Drinking Fountains - Restrooms will be sanitized/cleaned with a disinfectant cleaner. Wiping of surfaces with a disposable wipe will be utilized to maintain high hygiene levels. Where disposable wipes are not used then clean color coded micro fiber cloths will be used. Red cloths will only be used in toilets, toilet sinks and restrooms, green cloths are only for use in kitchens while blue cloths is only used in all other areas. Fixtures will be clean, polished, lustrous and free of encrustation, dust, mold, mildew, and streaks. Floors will be clean and free of dust, dirt, or debris. Recommended cleaning mop and chemical detergent depending on the type of the floor will be used. There will be no buildup in corners or along walls. Partitions, doors, windows, vents, sills, and walls will be free of obvious dust, dirt, bodily fluid and waste, and graffiti. Toilet supplies will be replenished to maintain an adequate supply during occupied hours. All sanitary napkin disposal containers will have the proper wax lined bag. Used waxed lined bags will be removed and replenished. Restrooms will be free of all discarded material, and trash will be emptied. @@Company name is responsible for furnishing and installing paper towel, toilet paper, soap, and toilet seat covers. Drinking fountains will be free of dirt, watermarks and all other debris and encrustation and have a lustrous appearance. Wiping of surfaces with a disposable wipe will be utilized to maintain high hygiene levels. Where disposable wipes are not used then clean blue colored micro fiber cloths will be used. Drinking fountains will be maintained at a high level of sanitation.
4.2.5 Interior Surfaces – Wiping of surfaces with a disposable wipe will be utilized to maintain high hygiene levels. Where disposable wipes are not used then clean blue colored micro fiber cloths will be used. Red cloths will only be used in toilets, toilet sinks and bathrooms, green cloths are only for use in kitchens. All vertical surfaces 70 inches and below (metal finishes, door knobs, walls, cove base/baseboards, doors, door jambs, picture frames, light switch plates, glass surfaces, [excluding exterior windows] interior room windows, mirrors, gym mirrors, etc.) and horizontal surfaces, (desks, countertops, picture frames, filing cabinets, etc.) will have no accumulation of dust, dirt, marks, hand marks, watermarks, streaks, smudges, fingerprints, encrustation, and soil substances. Cabinets and desks with papers, computers and keyboards will not be disturbed. All vinyl surfaces of fitness equipment will be free of dust, dirt, debris, streaks and smudges. Wiping of surfaces with a disposable wipe will be utilized to maintain high hygiene levels. High Cleaning is defined as surfaces that are 70 inches or more from the floor surface. These surfaces shall be cleaned and free of dirt, dust, and cobwebs. Where glass is present, both sides shall be clean and free of streaks. This includes, but is not limited to: dusting walls, ventilation and air conditioning outlets, transoms, clocks, moldings around ceilings, tops of partitions, overhead pipes, pictures, plaques, wall or ceiling diffusers, file cases, bookcases, and lockers. This does not include the removal of vents, tiles, or fixtures. @@Company name will provide their own ladders, step stools or other lift devices as necessary to complete these requirements. Any such equipment owned by GSA or tenant agencies cannot be used by @@Company name under any circumstances. High Cleaning is an above standard service and ordered using the relevant Hourly Labor Rate pricing in Section B. Elevators will be free of finger marks, smudges and spills and door tracks will be free of dirt, debris and build up grime.
4.2.6 Grounds and Exterior – Grounds, planted areas, entranceways, steps, landings, sidewalks, parking areas, driveways, loading docks, garages and hard surfaces: These outside locations will be policed for litter, debris, paper, gum and other discarded material daily. Parking lot and loading dock will be free of oil. Daily policing of the garage will be done, and litter and debris removed. Sweeping and policing of garage door openings and elevators will be accomplished as well. Ash trays and receptacles will be emptied and clean. Ash receptacle will be replenished, as needed, with sand like material designed for ash trays. Excrement will be removed from stairs, entrances, and sidewalks. Landscape areas will be well maintained and free of weeds and trash. Outside walkways, landings, and steps will be free of debris and weeds. Items included in landscape maintenance includes soil aeration, watering, pruning, mowing, trimming, raking, weed control, mulching and policing to keep the area clean and free of debris. Fertilization of lawns and trees is to be accomplished as follows:
4.2.6.1 Mowing and Trimming – Turf areas will be kept neatly cut and trimmed. During periods of slow growth, mowing will be scheduled on an as needed basis or when general turf growth exceeds half again the specified mowing height between cuts. Mowing height will be maintained at approximately 3 inches. Lawn areas will be kept free of trash and debris and clippings will be removed from sidewalks and paved areas. String trimming will be performed every visit to maintain a clean crisp appearance.
4.2.6.2 Sidewalk & Curb Edging – All curbs and sidewalks will be blade-edged as needed to maintain a crisp edge.
4.2.6.3 Pest Management Plan - An Integrated Pest Management Plan will be implemented to hinder disease and pest infestation (Exhibit #2). Pest and disease control will be completed for lawns, flowers, trees and shrubs as indicated in the above schedule. @@Company name will be held liable for any damage to grounds, vegetation, or landscaping due to our negligence. Leaf removal will be done to remove accumulations and storm drains will be checked/clear of leaf and debris accumulation.
4.2.7 - Pest Management - @@Contractor name will furnish all supervision, labor, material, and equipment necessary to accomplish the monitoring, trapping, pesticide application, and pest removal as components of @@Building name Pest Control Plan. @@Company name will adequately suppress indoor populations of rodents, insects, arachnids, cockroaches, ants, flies, and other arthropod pests not specifically excluded from the contract. This includes outdoor populations of potentially indoor-infesting species that are within the property boundaries of the specified buildings and individuals of all excluded pest populations that are incidental invaders inside the specified buildings, including winged, swarmer termite, emerging indoors. Twice a year an exterior barrier treatment will be applied to limit incidental invaders. Populations of the following pests are excluded from this contract: Birds, bats, snakes, bed bugs and all other vertebrates other than rodents; Termites and other wood-destroying organisms. @@Company name will employ techniques to discourage pest integration.
4.2.8 Snow and Ice Events - @@Company name contract employees will report to the @@Building name for clearing and treating snow/ice for applicable areas. Snow and ice will be manually removed from the entrance ways, steps, landings and sidewalks before the building occupants report for work @@7:00 a.m. and maintained during business hours @@5:00 p.m. Any snow removal requiring mechanized equipment such as snow blower, pick-up truck with snow blade, and etc. is an above standard service and will be completed and billed separately (Ordered using the relevant Snow Removal Rate pricing in Section B). Snow Plowing service will be implemented with a snow fall of 2 inches or more for sidewalks, parking lots, driveways, and vehicular courts. Snow removal service is required Monday thru Friday during regular business hours @@7:00 a.m. to 5:00 pm. and/or as directed by the COR. Chemicals and/or sand will be used to reduce safety hazards due to ice and snow. All chemicals used will be in accordance with Federal Specifications, local codes, and as approved by the COR. Caution will be used in the removal and placement of snow.
4.2.9 Additional Standard and Above Standard Services -
4.2.9.1 Stripping and waxing floors-Standard - (Exhibit #3)
4.2.9.2 Blind Cleaning – Standard - Blind cleaning for public spaces, lobby, restrooms and hallways will be completed.
4.2.9.3 Window Washing - Above Standard - Window washing is an Above Standard Service and will be completed and coordinated with COR upon request. (Ordered using the relevant Window Washing pricing in Section B). Glass shall be clean and free of grime, streaks and moisture. Interior glass sills and sashes shall be wiped free of drippings and other watermarks.
4.2.9.4 Carpet Cleaning- Standard - Carpet cleaning under the service is limited to common spaces only including GSA Property Manager’s Offices. Moving of all types of electronic equipment will be coordinated with the COR prior to cleaning the carpet. There will be no evidence of deterioration or fuzzing from harsh scrubbing or brushing. Cleaned areas should blend with adjacent areas. Provisions will be made so the carpet is dried to minimize the potential for mold and mildew growth.
4.3 Inspection Plan and Checklist – Inspection plan/Checklist: @@Company name Project Manager will conduct inspections. @@Company name PM will conduct bi-monthly inspections. 1st inspection will be completed during first two full weeks of a normal work month. 2nd inspection will be completed during last 2 weeks of normal work month. Days will fluctuate to ensure consistent daily quality. For the services that do not meet the contract requirement, rework will be initiated. Follow up inspections will be required after a corrective action is completed to ensure the correction was effective. The Inspection Checklist can be found in Exhibit #4.
4.4 Quality Control Forms and Reports - Quality control consists of the processes of 1.Quality analysis, 2. Quality assessment, and 3.Quality measuring system. The CM will conduct QC inspections in month and month as a minimum and more often if needed. Submit quarterly to the COR a self-evaluation report (The purpose of the self-evaluation process is to highlight strengths, correct performance weaknesses, and develop unused skills and abilities) Exhibit #5 detailing the quality of service provided during the prior quarter. The report is due within 5 business days of the end of the quarter. This report shall include as a minimum the result of any quality control inspections, an explanation of efforts taken in the prior quarter to improve service and efforts planned for the present quarter to improve quality. For the services that do not meet the contract requirement, rework will be initiated. Our QC inspectors will utilize quality control forms and reports to ensure ongoing monitoring for quality assurance, measuring results and reporting performance and trends. Reports will document deficiencies and the corrective actions custodians are taking, or will take, to eliminate the deficiencies. Custodial deficiency reports will contain information such as the area or space, date inspected and items on the cleaning procedure checklist. The report will use a pass or fail rating system. Tenant survey results – @@Company name plans to utilize the Government’s tenant survey results to improve the delivery of custodial and related services. We will work with the GSA Property Manager by reviewing each score and comment looking for opportunities to enhance/improve the services our company provides. We will draft a plan for the GSA Property Manager to approve within 30 days of our meeting.
4.5 Minimum Performance Quality Levels Standards - Cleanliness Standards: our QCP uses a ratings standard for cleanliness of Pass/Fail. A pass rating would meet criteria such as "floor covering bright and clean," "no dust on vertical surfaces," "some containers have a little waste" or "some dust accumulation on vertical surfaces." A fail rating would meet criteria such as “dirt, dust and buildup in corners on the floor covering.” Our QC Inspectors will assess the effectiveness of our Quality Control Program through building inspections, review of reports required by our contract, service delivery information, customer interaction, and by means of GSA inspection reports.
4.6 Equipment - “Cleaning personnel cannot clean efficiently and well without the proper equipment, tools and cleaning products to help them do good work”. All custodial equipment will be maintained properly. This maintenance ensures equipment safety. Types of equipment include custodial carts (in good repair, clean, organized, stocked with supplies), mops (changed weekly: if white when new the mop head should be close to that color if cleaned weekly, strings should not be missing or severely worn and odor free), brooms (broom's condition will be judged visually. If it looks like it has clearly seen better days, it's time to replace it) and mop buckets (mop buckets will be clean and not have water left overnight in them). Power equipment consist of vacuum cleaners (vacuum cleaner beater bars will be replaced if the brushes do not touch a straight edge, HEPA filters will be replaced on a fixed annual schedule, vacuum cleaner bags will be replaced as needed or monthly at a minimum), floor buffers/auto-scrubbers (pad bristles will be straight), carpet extractors (clean and free of leaks). The Supervisor will instruct custodians to clean the equipment after each use, and insure it is cleaned/maintained. Also, supervisors/custodians will inspect power equipment before each use and evaluate power cords to identify any frayed wiring or loose or broken plugs ((all electrical cords will be free of damage (tape, cuts)); all electrical cord plugs will have three prong plugs). Custodians will not use equipment in need of repair and will notify the supervisor of equipment in such need. Supervisor will remove equipment from the site immediately until repairs are properly made. Equipment maintenance log (vacuum HEPA filter replacement) will be maintained and available upon request.
Section 5 – Service Calls Communication Plan - @@Company name will use technology (e-mail, cellular phone) to communicate with GSA COR to receive and respond to service calls and emergencies. A supervisor or designee will be available to receive requests, notices and reports during building operating hours. Service calls will be created/monitored using NCMMS and responded to in a timely manner. The NCMMS Service Call Report will be submitted at the end of each month to the COR. For Emergency after Hour Service calls contact (xxx) xxx-xxxx.
Section 6 – Documentation, Submittals and Deliverables Task and Frequencies – Exhibit #1 Integrated Pest Management Plan – Exhibit #2 Floor Maintenance Schedule – Exhibit #3 Cleaning Inspection Checklist – Exhibit #4 QC Self-evaluation Report – Exhibit #5 Section 7 – Employee Training @@Company name believes that productivity and high morale are closely linked to a good training program, and a well-trained staff is essential to an environment for continuous quality improvement. Our training will take place on site. The training program is designed to ensure that all personnel are capable of performing the tasks to which they are assigned. It is also designed to monitor our own progress by continually improving upon or enhancing work methods. Effective training plays an important role in making it clear that all areas demand the same level of cleaning at all times. Our Training Program is a continual, on-going process, dealing with several components:
· Determining competency requirements,
· Identifying training needs,
· Providing training,
· Evaluating the effectiveness of training,
· Ensuring quality awareness,
· Maintaining training records,
· Equipment Maintenance,
· Customer satisfaction/communications,
· Personal appearance,
· Continual improvement,
· Training opportunities,
· Safety Data Sheet (SDS): Training will be on-site provided annually in month. Topics to be discuss include properties of each chemical; the physical, health, and environmental health hazards; protective measures; and safety precautions for handling, storing, and transporting the chemical. All personnel will be trained prior to using a newly introduced chemical. The Project Manager will provide the SDS training and document.
· Blood Borne Pathogens: Training will be on-site provided annually in month to all employees and Hepatitis B shots are offered free of charge to all employees for their protection. The Project Manager will provide the Blood Borne Pathogens training and document.
· Custodial Training upon hiring, @@Company name will provide on-site custodial training to staff. The Project Manager will provide the training and document.
· Asbestos Awareness Training - @@Company name will provide on-site asbestos awareness training and the facility/building manager will identify any known areas that contain asbestos to the contractor. Asbestos Awareness training will be conducted annually in month and documentation submitted to COR within 5 days of completion of training.
· Green Cleaning Training (Stewardship): Green cleaning practices will be implemented on this custodial contract. All new staff will be trained on-site in cleaning using green cleaning practices. The Project Manager will provide Green Cleaning training and document.
· Safety meetings/ trainings: @@Company name will provide on-site safety training to staff every other month and ongoing training will be provided as needed to keep employees informed of safety procedures, they need to follow to perform their jobs using the best safety practices. The Project Manager will provide Safety meeting/training and maintain attendance logs.
· Safety Equipment and Personal Protective Equipment: @@Company name will provide all the necessary safety equipment and personal protective wear for their employees. Items that will be provided include, but are not limited to: rubber/latex gloves, protective eyewear, hearing protection, hats, ice cleats, and other recommended items that would assist in accomplishing the prescribed work.
Section 8 – Contractor Emergency Plan (CEP)
8.1 Communication Procedures - @@Company name, Inc. will communicate any emergency plan with all their personnel by talking directly to them or by using cellular telephones. @@Company name personnel have been informed to remain calm, alert and listen for information and instructions from, C.S.O personnel, Homeland Security personnel, U.S. Marshall Personnel, local emergency crews or local officials.
8.2 Fire or Bomb Threat - Evacuation Procedures are posted on building maps with exits at several locations. All Exits are clearly marked and @@Company name personnel have been instructed to utilize the closest, safe exit. The Fire Alarm will sound during bomb threat evacuation.
8.3 Tornado/Severe Thunderstorm Warning - @@Company name personnel have been directed to go directly to an enclosed windowless area in the center of the building in which they are located. Personnel will be advised to; report to the basement area, stay away from windows and interior glass, crouch down and cover head once imminent impact is determined; stay off elevator, as to not get trapped if power is lost.

8.5 Employee’s Duties - The Project Manager, or the staff person in charge, will account for all personnel on site.
Section 9 – Strike Contingency Plan
In the event of an extended worker absence, the contract would be staffed by outlying experienced custodial/supervisory staff and associates of @@Company name working the necessary hours, including overtime to fulfill contract requirements. @@Company name requires that all custodial staff receive training upon being hired on all contracts thus all custodial staff from outlying locations will be experienced in general custodial cleaning. In order to gain the experience necessary for meeting the requirements of this contract and have access to areas, any custodial staff brought in, will work under the direction of a custodial staff person that is already assigned to the contract. In the event of a work stoppage, qualified personnel on other contracts would be summoned to report to work on the site immediately. These people work at other government contracts and already have security background clearances. Additionally, corporate and regional management personnel would be prepared to perform “hands on” tasks if needed. Regional personnel can be on-site within the same day. Corporate personnel can be on-site within 24 hours. Our company maintains a vast personnel file of resumes and applications in which it relies. Replacement personnel will have the proper licenses and certifications necessary for the positions they are filling, and will meet security clearance requirements. In the event of work stoppage, the following steps would be taken immediately, and throughout the period, until the resolution of the work stoppage.
1. Suspend approval of all vacations, with cancellation of all approved vacations.
2. Review application files at the State Employment Office, as well as our files to prepare listings of names and phone numbers of qualified personnel.
3. Our labor relations specialists will arrive to coordinate temporary replacement of personnel and review facilities.
4. Ensure that all critical non-striking personnel are prepared to assume duties outlined in this plan.
5. Identify local contractors capable of supplementing our contingency work force in the event of mission critical support requirements, including “temp” agencies. If any temporary or subcontractor employees are required for assistance, those personnel will possess the proper licenses and certifications and will have met the proper security clearance requirements
6. Advise employees that company paid benefits for all strikers will be terminated.
7. Contact the company attorney and begin liaison to assure legality of all company actions.
8. Notify the CO/COR that we intend to hire replacements for the strikers.
9. Finalize preparation for interviewing and processing personnel, including preparation of personnel folders with all necessary work.
10. Deliver employment advertisements to local newspapers and internet job posting services to be put on hold pending notice to run.
11. Provide State Unemployment Compensation Office with a list of striking employees to ensure that employment compensation is disallowed during the work stoppage period.
12. Begin hiring replacements if work stoppage continues.

Pests to be Controlled: Pest control services which can be performed in the General Pest Control category includes cockroaches, ants (other than carpenter ants), winged termite swarmers emerging indoors, incidental/occasional invaders including bees & wasps entering from out of doors, and flies and other arthropod pests. Populations of these pests that are located immediately outside of a specified building and pose a possible infestation problem to that building are included.
General: @@Building name will be inspected by @@Name of the pest control company (PCO) for the purpose of identifying potential problem areas that may be contributing to pest infestation within the facility, making recommendations for corrective measures that should be implemented. This IPM plan will utilize all methods of pest control, which may include structural maintenance, sanitation, monitoring for pest populations, mechanical and biological control, and the judicious use of pesticides. These methods will help to eliminate food, moisture and harborage for pests, making their survival more difficult. Pesticides will not be applied on a routine basis; however, they may be used as a tool to eliminate/maintain pest populations. The selection of pesticides that may be used will be based on a pre-determined hierarchy, which will utilize least toxic products as first choice. Proper implementation of this program will reduce the volume, toxicity and frequency of application of pesticides, thereby reducing the risk of potential exposure of building occupants who may be sensitive to their use.
The PCO and COR shall meet to discuss areas that have been problematic or sensitive. Areas that are sensitive to pesticide use will also be discussed. (i.e.; child care areas, work area of sensitive employees, etc.) Once these areas have been identified, the PCO and COR will discuss various pest control options.
Recommendations: @@Name of the pest control company will submit recommendations for corrective measures in writing to the COR prior to the application of any pesticides. The COR is responsible for notifying the building tenants and scheduling recommendations as soon as possible. A member of the custodial staff will escort to allow the pest control technician to access areas that may be locked.
Records: The pest control technician will indicate pest problem areas and provide written recommendations for structural, sanitary or procedural modifications on "Pest Control Service Record and Pest Inspection Report" forms or substantially similar substitute. These forms will be kept in a file that will be maintained in the Custodial office. All pest sightings should be reported in the logs and should include specific information as to the location and type of pest, if known.
Monitoring: Glueboards/Sticky Traps are basic method to monitor the environment for pests. Glueboards/Sticky traps can be used initially to determine if pests are present and the species. Glueboards/Sticky traps are not a means of mitigation, but detection and identification. Glueboards/Sticky will be used for the duration of the IPM program to monitor pest populations and activity. They will be placed in areas where pest activity has been identified or is likely to occur. The dates of installation and servicing will be indicated on each monitor and the pest control technician will create diagrams or maps indicating their placement. The diagrams will be maintained on site. Visual inspections of the glueboards/sticky traps will help the pest control technician to identify specific areas of infestation, if any, and assess the need for further action. Glueboards/sticky traps will be replaced monthly or when dirty/contain captured pests.
