About this file

This is a Past Performance Questionnaire template for the Architect of the Capitol (AOC) solicitation AOCSSB26R0002 for Senate Food and Related Services. The questionnaire is completed by previous clients or contract administrators who have worked with a contractor bidding on the AOC opportunity, and responses are to be submitted directly to the AOC Contracting Officer Moyah Wilson (moyah.wilson@aoc.gov) and Andrew Eichhorn (andrew.eichhorn@aoc.gov) by the proposal submission deadline of Wednesday, June 24, 2026 at 12:00 pm ET.

The questionnaire evaluates contractor performance across 16 categories using a four-tier rating system: Outstanding (O), Satisfactory (S), Marginal (M), and Unsatisfactory (U). Evaluation areas include schedule performance, quality control, compliance with labor and safety standards, pricing reasonableness, relationship with government teams, on-site management, corporate integrity, contract modifications, and overall satisfaction with the end product. Respondents must provide specific supporting information, particularly for outstanding or unsatisfactory ratings, and identify whether cure notices, terminations, or other corrective actions were issued. The questionnaire also requires respondents to indicate whether they would award another contract to the same contractor and allows space for additional comments. Responses are designated as Source Selection Sensitive and are protected under FAR 42.1503, limiting distribution to government personnel involved in source selection evaluation.
