About this file

The document is a U.S. Department of Housing and Urban Development (HUD) form HUD-51004, which is a Schedule of Materials Stored form used for documenting material costs in public housing construction projects. The form is designed to track and validate the value of materials stored at a project site, with specific instructions for contractors to summarize material values, with payments limited to 90% of the estimated material value. The form is part of the documentation process for partial payment estimates in public housing development projects.

The form includes provisions for detailed reporting by general contractors and subcontractors, requiring precise documentation of material costs, storage location verification, and certification by authorized representatives. It is part of HUD's oversight mechanism to ensure development costs remain consistent with construction requirements, with a warning that false claims may result in criminal and/or civil penalties. The form is approved under OMB Control No. 2577-0157 and is valid through 11/30/2023, supporting HUD's regulatory responsibilities under Section 6(c) of the U.S. Housing Act of 1937.
