When performance thresholds are exceeded, the Contracting Officer will issue a Contract Discrepancy Report (CDR) requiring written response from the service provider within 15 calendar days of receipt.

PERFORMANCE REQUIREMENTS SUMMARY (PRS)

1.0 SERVICE SUMMARY (SS)

1.1 PERFORMANCE OBJECTIVES: Column 1: The “Performance Objective” describes what the Government will survey. The absence of any contract requirements from the PRS shall not detract from its enforceability nor limit the rights or remedies of the Government under any other provision of the contract, including the clauses entitled “Inspection and Acceptance”, “Termination for Convenience”, or “Termination for Cause” contained in

Federal Acquisition Regulation clause 52.212-4. The Government has the right to inspect all services required in the contract.

1.2 PERFORMANCE WORK STATEMENT (PWS) REFERENCE: Column 2: The “PWS Reference” specifies where the “Performance Objective” is in the PWS.

1.3 PERFORMANCE THRESHOLD: Column 3: This column sets forth the “Performance Threshold” (PT) for each “Performance Objective” that may occur before the Government may take remedial action.

2.0 GOVERNMENT QUALITY ASSURANCE: Service provider performance shall be compared to the contract standards and PRS. The Government may use a variety of surveillance methods to evaluate the service provider’s performance to determine if it meets the contract standards. Only one method will be used at a time to evaluate a listed service during an inspection period for payment computation purposes. The methods of surveillance that may be used are:

2.1 One hundred percent surveillance of the output items.

2.2 Periodic surveillance of output items (daily, weekly, monthly, quarterly, semiannually, or annually) as determined necessary to ensure a sufficient evaluation of service provider performance.

2.3 Customer complaints.

3.0 PERFORMANCE EVALUATION: Performance of a service will be evaluated to determine whether it meets the Performance Threshold of the contract. When the Performance Threshold is exceeded, the CO shall issue a

Contract Discrepancy Report (CDR) to the service provider. The service provider must provide a written response to the CDR within 15 calendar days of receipt.
