About this file
This is a memorandum from the Department of Defense Defense Health Agency at U.S. Naval Hospital in Guam establishing procedures for administering and reviewing Nutrition Care Patient and Guest Satisfaction Surveys. The memorandum is directed to all Nutrition Care Staff and outlines policies and responsibilities for continuous quality improvement in food service and dietary support operations.
The surveys will be distributed to both inpatient and outpatient patients and guests receiving nutrition care services, with responses kept anonymous to encourage honest feedback. Completed surveys will be collected daily and reviewed weekly by the Nutrition Care leadership team, with trends, concerns, and commendations documented and addressed during monthly performance review meetings. Responsibilities are distributed among three groups: Nutrition Care Staff will ensure surveys are offered and explain their purpose; the Section Supervisor will oversee collection, secure storage, and timely submission of survey data; and the Quality Improvement Officer will analyze results and recommend corrective actions or enhancements. The memorandum addresses meal quality, service timeliness, staff courtesy, and overall dining experience as key feedback areas.
