QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
1. Contract or Task Order Title. Building Automation System (BAS) Installation & Maintenance Enterprise-wide Services
2. Work Requirements:
· Task 6.1 – Provide Preventative Maintenance Services
· Task 6.2 – Provide Application Administration Services
· Task 6.3 – Provide Ongoing System Documentation
· Task 6.4 – Provide Enterprise Supportworx – Iconics Support
· Task 6.5 – Unscheduled Maintenance, Repair, and Replacement Parts
· Task 6.6 – Emergency Onsite Service Calls
· Task 6.7 – Systems Repairs and Replacements, Modifications and Additions (Technical Requirements)
· Task 6.8 – Provide New BAS Installations
· Task 6.9 – Provide Consulting/Design of New BAS
3. Primary Method of Surveillance: See method of surveillance for specific tasks under Section 5 - Performance Standards.
· 100 Percent Inspection. This is usually only the most appropriate method for infrequent tasks or tasks with stringent performance requirements, e.g., where safety or health is a concern. With this method, performance is inspected/evaluated at each occurrence. One hundred percent inspection is too expensive to be used in most cases.
· Random Sampling. This is usually the most appropriate method for recurring tasks. With random sampling, services are sampled to determine if the level of performance is acceptable. Random sampling works best when the number of instances of the services being performed is very large and a statistically valid sample can be obtained. Computer programs may be available to assist in establishing sampling procedures.
· Periodic Inspection: These services are monitored weekly, monthly, quarterly, semiannually, annually, etc. Periodic types of activities are perfect for periodic inspection because not only are they infrequent, but there is normally a predetermined, specified time frame within which the tasks must be accomplished. Therefore, you know exactly when to conduct the evaluations. Periodic inspections automatically become 100 percent evaluations or "100 percent checks." Inspections should be divided and scheduled by frequency: annual, semiannual, quarterly, monthly, weekly and as required. Sometimes services are required for which the time or frequency cannot be predicted, such as accident investigations, one-time special tasking by higher headquarters, etc. These would be labeled "as required inspections." Others are known and predictable such as the quarterly status report or the monthly travel report currently included in some DISA service contracts.
· Customer Input. Although usually not a primary method, this is a valuable supplement to more systematic methods. For example, in a case where random sampling indicates unsatisfactory service, customer complaints can be used as substantiating evidence. In certain situations where customers can be relied upon to complain consistently when the quality of performance is poor, e.g., dining facilities, building services, customer surveys and customer complaints may be a primary surveillance method, and customer satisfaction an appropriate performance standard. In all cases, complaints should be documented, preferably on a standard form.
The facility manager or alternate facility representative should know when Contractor personnel are performing services in support of the facility, whether routine, emergency, or in support of a Technical Requirement (TR). The Contractor personnel shall sign in to the supported facility IAW established security and visitor protocols, and shall physically notify the facility manager or alternate facility representative of their arrival and departure. As a QA representative for the Government, the project engineer or facility manager may, for any service visit, request the Contractor personnel to demonstrate what work was accomplished during the visit, discuss the condition of the serviced equipment, and/or inspect with the Contractor personnel the services rendered for compliance with guidance and instructions for the specific services. All demonstration/inspections shall be documented with a signed QA form.
Note: Expect to receive GSA approval this week on the updated QA Surveillance Form.
4. Scope of Performance:
The contractor shall provide maintenance and installation services, onsite application administration support, O&M manual updates, enterprise level support from Iconics, contract management and quality control, technical assistance, training, and certifications, accreditations, and authorizations, all within specified timeframes.
Maintenance is defined to include service, including parts replacement, as necessary to restore equipment that is in an inoperable or degraded condition to normal operating effectiveness. The contract reference is Section 5.

6. Mandatory or Regulatory Compliance Items:
The contractor shall provide initial proof of compliance with regulatory and compliance items contained in the contract. The Contracting Officer’s Representative (COR) and the contracting officer (KO) will evaluate the initial submission. The COR and KO will conduct ad hoc reviews of the contractor’s compliance throughout the remainder of the contract.
7. Acceptable Quality Level (AQL):
Majority of AQLs are 100% due to the critical support provided by the BAS. See Section 5 - Performance Standards for specific AQLs.
8. Evaluation Method:
The COR will document the time of verbal notification to the contractor. The on-site government representative will document and notify the COR and the contractor on the following:
· Random sampling of Preventive Maintenance and 100% inspect of the summary reports of Preventive Maintenance (PM) performed and notification of all firmware updates.
· 100% inspection of all Emergency Maintenance Services.
· Periodic inspection of all system repair and replacements, modifications and additions.
· Periodic inspection of all new system installations
· Periodic inspection of all operation and maintenance documentation.
· Site government representative will acknowledge and concur on a QA Surveillance Form with 100% inspection on all technical requirements work performed for realignment, additions and modifications to security system equipment.
The COR will review self-diagnostic systems logs, conduct a comparison with actual maintenance performance and otherwise verify and validate contractor performance. The contractor shall enter in the record the official time the system is restored to full operational status. The COR will confirm the date and time of problem resolution in the record.
9. Incentives (Positive and/or Negative): Both positive and negative performance will be documented in past performance reports/database, (Contractor Performance Assessment Reporting System (CPARS), as appropriate and will be taken into consideration for follow-on work.
