About this file
This is a Vendor Declaration Statement form issued by Collier County Board of County Commissioners Procurement Services Division in Florida for Request for Proposal (RFP) No. 25-8334 seeking managed services and maintenance for a comprehensive P25 public safety radio system. The solicitation encompasses maintenance and support services for mission-critical 800MHz radio infrastructure serving approximately 11,000 public safety and governmental users across five counties, covering nearly 2,300 square miles. The system includes 15 tower sites, fixed network infrastructure, terminal equipment, and L3-Harris radio equipment requiring preventive and corrective maintenance, event monitoring, software updates, security maintenance, emergency response capabilities, and terminal equipment rental programs. A pre-proposal conference and mandatory site visits are scheduled for November 21, 2025, with proposals due on December 10, 2025 at 3:00 PM EST. The contract is structured for an initial ten-year term with two optional five-year renewals, providing potential total contract value exceeding $3.5 million.
The form requires vendors to declare that their bid submission is made without collusion, fraud, or arrangement with other parties, and in compliance with Collier County's Procurement Ordinance ethical standards. Vendors must certify no financial conflicts of interest exist with county elected or appointed officials, with financial interest defined as ownership exceeding one percent of total assets or capital stock, or any officer, director, manager, partner, proprietor, or agent role. Historical spending by county departments on these services totals approximately $1,750,000, though this may not indicate future buying patterns. Vendors are required to meet compliance requirements including E-Verify program enrollment, immigration law compliance, and insurance coverage with most liability insurances set at $1,000,000 per occurrence. The submission must be completed on company letterhead with authorized signature and includes attestation under penalties of perjury.

The Vendor states that the submitted is based upon the documents listed in this solicitation. The Vendor agrees to comply with the requirements in accordance with the terms, conditions and specifications denoted herein and according to the pricing submitted as a part of the Vendor’s bids.
Further, the Vendor agrees that if awarded a contract for these goods and/or services and a subcontractor is a related entity to the Vendor, then the Vendor shall not mark-up the subcontractor’s fees. A related entity shall be defined as any parent or subsidiary of the company and any business, corporation, partnership, limited liability company or other entity in which the company or a parent or a subsidiary of the company holds any ownership interest, directly or indirectly.
Under penalties of perjury, I certify that the information shown on this form is correct to my knowledge and a duly corporate authorized representative with signature authority.
