The form certifies that prospective bidders and their principals and subcontractors are not debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in federal transactions by any Federal department or agency. Bidders must submit this certification as part of their bid package, with a mandatory pre-proposal meeting scheduled for Tuesday, March 31, 2026, at 10:00 a.m. at City Commission Chambers, 302 West Reynolds Street, Plant City, Florida 33563. Written questions must be submitted by Monday, April 13, 2026, at 5:00 p.m., and sealed bids are due Thursday, April 23, 2026, at 2:00 p.m. via the City's OpenGov E-Procurement Portal. The contract requires substantial completion within 90 calendar days of the Notice to Proceed, with work commencing within 14 calendar days thereafter, and all bids must remain valid for a minimum of 90 days from the submission deadline.
The certification must be notarized by a Florida notary public and signed by an authorized representative of the bidding company. Any prospective recipient of federal assistance funds that cannot certify compliance must attach an explanation to their response. The form constitutes a material representation of fact; knowingly rendering an erroneous certification may result in suspension, debarment, or other remedies available to the Federal Government and the Department of Labor. Bidders must provide immediate written notice if their certification becomes erroneous after submission due to changed circumstances. By submitting this response, bidders agree not to knowingly enter into lower tier covered transactions with debarred or excluded persons and must include identical debarment certification language in all subcontracts and lower tier solicitations. This certification is required under 2 C.F.R. Part 180, implementing Executive Order 12549 and Executive Order 12689.
