1. The purpose of the amendment is to:
a. Extend the "Offer Due Date/Time" to 12:00 pm Central Time (CT), 29 October 2020.
b. Schedule a site visit for 9:00 am CT, 21 October 2020.
c. Extend the “Questions” suspense to 10:00 am CT, 23 October 2020.
d. Update the “period of performance/work shall be completed” to 360 calendar days.
e. Sections B through M are amended based on several FAR provisions and clauses updates; included as “Attachment J-11

f. Update Attachment J-4 U.S. Department of Labor General Decision Number: AR20200166.
g. Update Attachment J-5 Sample Client Letter.

B. A site visit is scheduled for 9:00 AM (CT) on October 21, 2020. Offerors are highly encouraged to attend. Space is limited due to social distancing requirements. Each Contractor is limited to two attendees. Notification of intent to attend plus a completed "Attachment J10-Clinic Risk Assessment Questionnaire for Jefferson Labs (JL)" for each attendee must be sent to Jennifer Johnson at jennifer.johnson3@fda.hhs.gov no later than 7:00 AM (CT) on October 20, 2020.
C. Questions concerning the project are required in writing to jennifer.johnson3@fda.hhs.gov by 10:00 am (CT) on October 23, 2020.
D. The period of performance/work has changed from 270 calendar days to 365 calendar days.
E. Sections B through M have been amended based on several FAR provisions and clauses updates; included as “Attachment 11 – 75F40120R00012 Section B through M Updates”.
F. Attachment J-4 has been updated to U.S. Department of Labor General Decision Number: AR20200166, dated May 1, 2020 (5) Pages
G. Attachment J-5 Sample Client Letter has been updated to include the new “Response date/time”; no later than 12:00 pm CT, 29 October 2020.
H. L-7 Submission of Offers, Part I - Recent and Relevant Past Performance Information include the Prime Contractor and the Sub-Contractor ONLY if the sub contactor is performing the Improvements & Paving work.
I. L-7 Submission of Offers, Part II - Price Proposal Instructions:
i. The Contractor shall submit a Certification of Current Arkansas State Contractor’s License for the Prime
Contractor and the Sub-Contractor ONLY if the sub contactor is performing the Improvements & Paving work.
J. M-2 Evaluation Criteria
a. Amend 1st paragraph as follows:
The past performance evaluation will assess the relative risks associated with likelihood of success in performing the solicitation's requirements as indicated by the improvement & paving contractor (prime or sub-Contractor) record of recent and relevant past performance (i.e., what, when, how well, size, and complexity of projects). *If the Prime contractor is not performing the improvement & paving work, the past performance evaluation will assess their ““Responsibility”/ relative risks associated with likelihood of success in performing Contract Management; as well as verification of CPARS and
FAPIIS.
