About this file
This file is a Buy American Certificate form (FAR 52.225-2, October 2022 version) that serves as a compliance document for federal contractors submitting offers on government procurement opportunities. The certificate requires offerors to certify whether each end product is domestic or foreign, with specific documentation requirements for each category. Domestic end products must be identified, and those containing critical components must be separately listed by line item number. Foreign end products manufactured in the United States that do not qualify as domestic must be disclosed, along with an indication of whether they exceed 55 percent domestic content (except for commercially available off-the-shelf items). The form includes structured sections for offerors to list line item numbers, country of origin, and domestic content percentages, with the government reserving evaluation authority under Federal Acquisition Regulation Part 25 policies and procedures.
This certificate is associated with a Department of Veterans Affairs Sources Sought Notice (Solicitation Number 36C25926Q0872) issued by Veterans Integrated Service Network 19 for a Transcranial Magnetic Stimulation (TMS) machine to support Salt Lake City VA Medical Center. The notice is intended solely for market research purposes and explicitly states that no award will be made from responses received. Interested vendors must submit responses by October 1, 2026, at 12:00 p.m. Mountain Time to lynn.lim@va.gov, providing company information, point of contact details, delivery timeframes, country of origin, manufacturing details, and small business program eligibility status. The anticipated NAICS code is 334510 (Electrotherapeutic Apparatus Manufacturing), and responses should indicate whether equivalent domestic end products are available for any non-US manufactured items.
