About this file
This file is a Buy American Certificate template (FAR 52.225-2, October 2022) used in federal procurement to ensure compliance with domestic content requirements. The certificate requires offerors to certify that each end product is domestic unless specifically listed as a foreign end product. Offerors must separately identify line items for foreign end products manufactured in the United States that do not qualify as domestic, and for non-steel/iron foreign products, indicate whether they exceed 55 percent domestic content (unless they are COTS items). Additionally, offerors must list line item numbers for any domestic end products containing critical components as defined in FAR 25.105.
The certificate is associated with a Department of Veterans Affairs, Veterans Health Administration (VHA) Network Contracting Office 19 Sources Sought Notice for a Pulsed Field Ablation System at Salt Lake City VA Medical Center. This is a pre-solicitation market research inquiry with a response deadline of 12:00 pm MT, September 14, 2026, to be submitted to lynn.lim@va.gov. Respondents should provide company information, point of contact details, manufacturing location, delivery time, and small business classification eligibility. The anticipated NAICS code is 334510 (Electrotherapeutic Apparatus Manufacturing). The notice specifically requests information on whether equivalent domestic end products are available from U.S. manufacturers if the item is non-U.S. origin. No award will be made from this notice, and responses are for information and planning purposes only.
