APPENDIX A STATEMENT OF REQUIREMENTS (SOR) SUMMARY – OPTIONS/TANK LIST

The Primary tanks to be inspected for this requirement are listed below in the FY 22 Table 1.

The first set as a priority is listed in Table 1. These tanks are the most overdue per the Government’s inspection data records (See Appendix G for API 653 Inspection records on these tanks).

The tanks listed in Table 1 must be inspected no later than 30 June 2022. If those tanks have only minimal repairs and/or can be placed back in service right away without repairs, as discussed with the Government Stakeholder(s), the Air Force intends exercise options for the contractor to inspect the next set of four tanks the following year in the spring/summer of 2023, and then the last set of four tanks in the spring/summer of 2024,by no later than 30 June of each year. This decision will be directed by the CO pursuant to the Options clause, and will be based upon tank conditions, future repairs, fuel levels, weather, stakeholder input, base support personnel workload, etc.

1.0 TANK BACKGROUND AND FEATURES

Each of the four tanks listed in Table 1 are field erected, ASTs built in the early 1950s of the API 12c design built-in that era. The tanks are not equipped with a floating pan. All tanks are built with a unique cut and cover venting system without overflow vents. The tanks were last inspected by Enterprise Engineering, Inc in July 2008, API reports show a new floor was installed 1999-2000 timeframe. Some known conditions must be addressed on all three individually:

1. Shand and Jurs tape gauge condition

2. Condition and cleanliness of the Shand and Jurs Pressure/Vacuum Vents Coating.

Failures were identified in 2008 API reports

3. Establish a tank floor corrosion rate based upon previous API reports and new floor scan.

1.1 Tank specific Efforts

Contractor shall provide qualified team members with sufficient knowledge and expertise to perform the EEMUA 159 inspection and prepare a comprehensive report. See Table of Deliverables A013, A014, A014a, )

a. Verticality of Round Survey. These tanks do not currently have a floating pan installed. To ensure the tank is capable of receiving a floating pan in the future to accommodate a future floating pan design, the Contractor is to conduct a Verticality of Round Survey with sufficient freeboard for stakeholders to review any potential restriction of double wiper seal arrangement. See Table of Deliverables (A07d)

b. EEMUA 159 Report Contractor shall report and recommend repair options for mandatory and non-mandatory EEMUA 159 repair items into the EEMUA 159 report effort to establish compliance path for future construction agents. (See Table of Deliverables A014)

The Contractor shall provide a final EEUMA 159 certification “suitability for service” outlining anomalies identified and certify that tanks are compliant with EEMUA Standards for continued use. The Contractor shall inspect tank containment basin, include map of the findings, and cost estimate of repairs in report. The Contractor shall provide and maintain an inspection schedule. Perform an inspection IAW good engineering practices, installation approved Spill Prevention Control and Countermeasures (SPCC) Plans, NFPA 30, and UFGS 33 01 50.60 in compliance with 40 CFR 112, UFC 3-460-01, UFC 3-460-03, applicable industry standards, and any applicable federal, state, local, service and host nation requirements. This document will be delivered with the draft EEMUA 159 report submission to notify the government of tank status for use. See Table of Deliverables (A015).

2.2 Confined Space and Safety (A019)

Since tank entry is required, the Contractor shall comply with confined space guidelines.

A certified tank inspector will assess the condition of the AST and determine its suitability for continued service. The Contractor shall perform a confined space entry to inspect the tank bottom, interior appurtenances and piping. The Contractor shall submit a confined space entry permit to the installation for access.

The Contractor shall submit all plans, materials, waivers, and/or permits to the CO using AF IMT 3000, Material Approval Submittal. Limit one item per submittal unless otherwise authorized by the CO or COR. All submittals shall be tracked through the use of form AF66 Submittal Register.

The Contractor shall refer to 29 CFR 1910.146, Permit- Required Confined Spaces for the API Inspection. In addition, the Contractor shall provide the proper personal protective equipment (PPE) as required. The Contractor shall provide a Certified Industrial Hygienist (CIH), or Marine Chemist to certify each tank is Gas Free and Safe for Entry prior to Inspection Activities. Request Permits a minimum of 30 Calendar days prior to inspection activities. See Table of Deliverables A019

2.3 Engineer Specifications (not Applicable)

2.4 Containment Inspection Report (A016)

Visually inspect containment for ability to contain the entire contents of the tank in the event of catastrophic failure. Assess for cracks, settlement and deterioration. The Contractor shall record dimensions of cracks to include length (feet), depth (inches), and width (inches) and provide graph mapping the cracks with each panel numbered for clarity of crack location and sizes. Provide all proper documentation including, drawings, report findings and repair recommendations back to the CO and COR. See Table of Deliverables A016)

Appendix A

2.5 Inspection Team

The Contractor shall deploy their inspection team to initiate inspection actions at the earliest possible timeframe due to travel and weather conditions.

2.6 Summary of Findings (A013)

The Contractor shall attend one out-brief per tank and present the Summary of Findings to Government stakeholders the next business day after each EEMUA 159 inspection is completed and before departing site. Out-brief shall facilitate off-site attendee participation. Each tank shall be returned to service no later than the next business day after receipt of Government direction provided at the facility out-brief. (See Table of Deliverables A013) The Contractor shall submit all plans, materials, waivers, and/or permits to the CO using AF IMT 3000, Material Approval Submittal. Limit one item per submittal unless otherwise authorized by the CO. All submittals shall be tracked through the use of form AF66 Submittal Register.

2.7 Inspection Sequencing

The Contractor shall sequence cleaning and inspection to minimize the interruption to mission support for multiple tanks at a single installation. The Contractor shall coordinate with the installation POCs, CO, and COR to ensure the Contractor’s schedule does not conflict with mission requirements. The Contractor shall notify the CO, COR, and installation personnel a minimum of 30 calendar days prior to taking the first tank out-of-service, allowing time for installation personnel to drain the tank until the installed pumps suction has been broken if necessary. The Contractor shall coordinate with the installation Fuels Management and Civil Engineering POCs prior to inspection activities;

protect all tanks from the elements during all nonworking hours.

2.8 Submittals

Contractor responsibilities include efficient management of work, submittal of accurate deliverables IAW this instruction, adherence to project schedule which shall be provided and maintained by the Contractor to include timely identification and solution of impediments to successful project execution. The Contractor shall provide the schedule as part of the monthly report and as changes are made.

2.9 Fuel removal

The Contractor shall clean and vapor-free each tank. Fuels Management/WFM will assist in emptying tank contents. All on-spec fuel shall be removed by Fuels Management personnel via installed pumps. Upon pump suction loss, the Contractor shall be responsible to recover and return to storage any on-spec fuel from the tank once cleared by Fuels Management Team. Residual fuel shall be sampled by base fuels management flight to ensure fuel quality before returning to storage. Any remaining fuel/sludge shall be considered contaminated at, and only at, the discretion of the Fuels Management Team shall be removed and disposed of by the Contractor with close coordination of utilizing the two 10,000 gallon tanks at the weathering pit.

(Refer to Appendix B, Internal Tank Cleaning Requirements)

Appendix A

2.10 Nozzle Reinforcement/Repads
