RE: Limited Lead-Based Paint and Asbestos Containing Material Testing For USAFA Cadet Chapel Colorado Springs, Colorado Empirical Project Number 351592

Dear Mr. Reish:

Empirical Environmental, LLC (Empirical) is pleased to submit this letter report presenting the results of the limited lead-based paint (LBP) and asbestos containing material (ACM) testing performed for renovation activities for the Cadet Chapel at the United States Air Force Academy. This testing was performed on November 16th, 20th and 30th and Dec. 1st, 2nd, and 4th, 2015 by Ms. Sheila Sealander (Colorado Lead Risk Assessor Certification number 8950 and Colorado Asbestos Building Inspector number 8737). This testing was performed prior to renovation activities.

1.0 LEAD-BASED PAINT TESTING

LBP testing was performed using a Niton XLi-702a Series Lead Analyzer (Serial Number 14719) that utilizes X-Ray Fluorescence (XRF) technology. The XRF’s testing mode was set at K & L Spectra Mode during this testing and the assay (source) date was February 1, 2015. This type of sampling is non-destructive. Empirical calibrated the XRF according to manufacturer’s directions prior to and at the end of the testing.

Substrate correction is not recommended for this XRF instrument. The Niton XRF does not have inconclusive readings or ranges; therefore paint chip sampling is not necessary.

Limited Lead Based Paint And Asbestos Material Testing Cadet Chapel, USAFA

Empirical Environmental, LLC 2

It was reported that the renovation activities might include removal of paint on the structural steel. This steel was tested on November 16th and 30th. Reportedly renovation might also impact the structural components of the organ that are located in the loft on the south end of the Protestant (upstairs) chapel. These components were tested on December 4th, 2015. This testing was not intended to be a LBP Inspection or Risk Assessment as defined by the U.S. Department of Housing and Urban Development (HUD). Forty-seven XRF readings were taken during these testing events:

22 were for calibration purposes; six (6) were null or incomplete readings; and 19 were at selected locations at the subject site. Five (5) XRF readings were taken on the steel structures inside tetrahedrons M23E and U24E. These XRF readings were above the U.S. Environmental Protection Agency’s (EPA’s) standard for lead-based paint of 1.0 milligram per square centimeter (mg/cm2) and would be considered LBP. Twelve of these XRF readings were taken on the organ structural components and two (2) were taken on the railing in the organ loft. These XRF readings were below 1.0 mg/cm2 and would not be considered LBP.

The XRF readings and locations are enclosed in Table 1, Appendix A. The Lead Risk Assessor’s certification and Empirical’s Lead Firm certification are included in Appendix B.

2.0 ASBESTOS CONTAINING MATERIAL TESTING

Asbestos bulk samples were collected by Ms. Sheila Sealander, Colorado State Certified Asbestos Building Inspector (Certification Number 8737). A copy of Ms. Sealander’s certification is presented in Appendix B. Additionally, a copy of Empirical’s Asbestos Firm certification is presented in Appendix B.

The bulk samples were submitted to EMSL Analytical, Inc. (EMSL) in Denver, Colorado for analysis of asbestos utilizing Polarized Light Microscopy (PLM). EMSL is an accredited laboratory for the analysis of bulk asbestos samples and is a member of the National Voluntary Laboratory Accreditation Program (NVLAP #200828-0). EMSL’s NVLAP certification is presented in Appendix B. Quality assurance samples were collected on November 30th and sent to EMC laboratories in Phoenix, AZ. EMC’s NVLAP certification (101926-0) is presented in Appendix B. The laboratory results are presented in Appendix C. This report is based on the bulk sample analysis results. An inventory and list of samples is presented in Appendix D. Photographs of ACMs are presented in Appendix E.

Suspect materials are considered positive for asbestos if the materials contain greater that 1% asbestos. As required by the EPA’s National Emissions Standards for Hazardous Air Pollutants (NESHAP) regulations, point counting analysis should be performed if the analyst detects asbestos in the sample and estimates the amount by visual estimation to be less than 10%, or the owner or operator of the building may either assume the amount

Empirical Environmental, LLC 3 to be greater that 1% and treat the material as ACM or require the verification of the amount by point counting.

Forty samples were collected from inside the tetrahedrons. The sample nomenclature for the tetrahedron samples is as follows: V-WW-X-YY-ZZ where V indicates lower, middle or upper tetrahedron; WW indicates the tetrahedron; X indicates east or west side of the Chapel; YY indicates the homogenous area for the day of sampling; and ZZ indicates the consecutive sample number for the day. Tetrahedron numbers were used to identify the general location of the samples. U or H indicates the upper tetrahedrons which comprised of east and west sides. M indicates middle tetrahedrons, and L indicates lower tetrahedrons. The following tetrahedrons were accessed during this sampling event:

M24E, U23, M32E, L29E, L3E, L33E, L33W, M32W, M24W, L19W, L17W, L1W, and L1E.

Nine (9) samples were collected from the insulation in the lower tetrahedrons. The lower tetrahedrons were accessed from the plaster panels inside the Chapel with the exception of the north and south tetrahedrons (L1 and L33) which were accessed by exterior panels.

These lower tetrahedrons were reportedly abated prior to the lights being installed. An apparent sheetrock bulk head was placed in these tetrahedrons above the lights. The areas above the sheetrock were not accessed during this testing event and should be assumed to have ACM insulation. Six (6) samples of the insulation below the bulk heads were collected from L29E, L3E, L19W and L17W (sample numbers L29E-01-01, L3E-01- 02, L19W-01-01, L19W-01-02, L19W-01-03, and L17W-01-04). These samples were reported as not containing asbestos. Tetrahedrons L1E, L1W, L33E and L33W were accessed from a panel on the exterior. The lower portions of these tetrahedrons appeared to have been abated and did not have insulation on the metal panels. The upper portions did appear to have insulation. The southern tetrahedrons were accessed and samples were collected from the insulation (sample numbers L33E-02-03 and L33W-02-04). This insulation was reported as not containing asbestos. L1E and L1W were viewed but appeared to have a bulkhead. The areas above these bulkheads were not accessed.

Insulation was not observed in the lower areas. However, at the bottom of the tetrahedrons appeared to be spray-foam (see photos in Appendix D). Laying on top of this spray-foam in L1E was gray insulation which most likely had fallen from higher in the tetrahedron. This insulation was sampled (sample number L1W-03-06) and was reported to contain 20% Amosite asbestos. Please note, this sample was collected from the east tetrahedron and not the west as the sample number would indicate.

Debris was observed on the interior metal railing or beam on the door panel in L33E. This debris was sampled (sample number L33ER-03-05) and was reported as not containing asbestos.

The middle and upper tetrahedrons that were accessed appeared to have insulation on the metal panels. This insulation appeared to be fiberglass with the exception of the

Empirical Environmental, LLC 4 bottom third of tetrahedron M32W which had a gray spray-on insulation. Sixteen samples were collected from what appeared to be fiberglass insulation (sample numbers M24-01-

01, M24-01-02, H23-01-01, U23E-01-01, U23W-01-02, U23TOP-01-03, U23WF-01-04,

M32E-01-07, M32W-01-01, M24W-01-02, and M24W-01-03). This insulation was reported as not containing asbestos. Five (5) quality assurance samples were collected from the insulation (sample numbers U23E-01-01A, U23W-01-02A, U23TOP-01-03A, U23WF-01-04A, and M32E-01-07A). These samples were reported as not containing asbestos. One (1) sample was collected from the gray spray-on insulation in the bottom of M32W (sample number M32W-02-04). This insulation was reported to contain 30% Amosite asbestos.

Plaster was observed inside the tetrahedrons and made up the inner walls of the tetrahedrons. Eight (8) samples were collected from this plaster (sample numbers M24- 02-03, U23E-02-05, U23W-02-06, M32E-02-08, M32W-06-09, M32W-07-10, M24W-07- 11, and L17W-02-05) and were reported as not containing asbestos. Three (3) quality assurance samples were collected from the plaster (sample numbers U23E-02-05A, U23E-02-06A and M32E-02-08A). These samples were reported as not containing asbestos.

Black sealant was observed at the corners of the metal panels where the insulation had been dislodged. One (1) sample of black sealant was collected (sample number L33W- 04-06) and was reported to contain 10% Chrysotile asbestos.

A mastic and material like caulk were observed on a spacer in U23E. This mastic was sampled (sample number H23-02-02) and was reported as not containing asbestos.

Twenty-three samples were collected outside of the tetrahedrons. Sample nomenclature for these samples is as follows: XX-YY-ZZ where XX indicates the side the sample was collected from; YY indicates the homogeneous number; and ZZ indicates the consecutive sample number. Four (4) types of caulk were observed and sampled from the Chapel:

white caulk was collected from the exterior of M24W (sample number M24W-03-05) and on the north side at the bottom of the windows (N-05-08); silver caulk was collected from the exteriors of M32W (sample number M32W-04-06) and M24W (sample number M24W- 04-07); black caulk was collected from the exterior of tetrahedron M32W (sample number M32W-05-08), from the north side windows, southeast of the door (sample number N-06- 09), from the screw holes in the door panel on the south side (sample number SE-14-19), from inside the window in the southeast corner (sample number SWI-17-22), and from the interior window/frame junction at L30W (sample number L30WI-18-23); and gray caulk from the panel on U23E (sample number H23-03-03), from the exterior of M24W (sample number M24W-08-12) and from the northeast corner at the window/frame junction (sample number NE-10-15). These samples were reported as not containing asbestos.

Empirical Environmental, LLC 5

Two (2) types of window glazing were observed on the windows on the north and south sides of the Chapel. Light gray window glazing was observed on the north side east of the doors and sampled (sample number NE-07-10). This sample was reported as not containing asbestos. Dark gray window glazing was observed on the north and south sides. This glazing was sampled from the following locations: sample number NE-08-11 was collected from the north side east of the doors; sample number SW-08-12 was collected from the south side, west of the door; and sample number SE-08-13 was collected from the south side, east of the doors. These samples were reported with 2% Chrysotile asbestos.

Three (3) types of putty were observed and sampled. Putty was sampled from the exterior northeast window at the glass and frame junction (sample number NE-09-14). This sample was reported with 5% Chrysotile asbestos. The second putty was observed on the interior of the middle panel by the doors on the south side between the panel and the frame (sample number SM-15-20). This sample was reported to contain 4% Chrysotile asbestos. The third putty was observed inside the Chapel at L27W at the cross-member of by the stained glass (sample number L27WI-16-21). This sample was reported with 20% Chrysotile asbestos and 3% Amosite asbestos.

Panels were removed from the interior of the doors and panels on the south side to observe the core areas of the doors. The core was sampled from the east door and the sealant that held the removed panel in place was sampled (sample numbers SE-12-17 and SE-13-18, respectively). These samples were reported as not containing asbestos.

A gray sealant was observed at the bottom of the on the windows on the north side. This sealant was sampled (sample number N-04-07) and was reported as not containing asbestos. A black window holder was observed on the low west window on the north side.

This material was sampled (sample number NW-11-16) and was reported as not containing asbestos.

These sample results were compared to the asbestos survey performed in 2004 by Maxim Technologies, Inc. The results of the materials tested in this event appear to confirm the results of the 2004 event.

The asbestos laboratory results and chain-of-custodies are presented in Appendix C.

Photographs of ACMs are presented in Appendix E. Site photographs are presented in Appendix F.

3.0 CONCLUSIONS

Upon completion of the limited LBP and ACM testing and review of the sample results, LBP was detected on the steel structures in the tetrahedrons but was not detected in the

Empirical Environmental, LLC 6 area behind the organ in the loft. ACM’s were reported in the insulation in the bottom of M32W, dislodged insulation in L1E, the black sealant at the seams of the tetrahedron panels on the interior, in the window putties, and the dark gray window glazing.

4.0 LIMITATIONS

Empirical has performed limited LBP and ACM testing in select tetrahedrons, and on select materials that were reportedly going to be impacted during renovation activities at the Cadet Chapel at the United States Air Force Academy in Colorado Springs, Colorado. Results presented in this report are based on XRF results and analytical data obtained for specific sampling locations at the time of this testing event. Other sampling data at a different time or location may provide different results. Empirical used XRF results to determine if paint had lead and analytical results to assess whether ACMs were present in the samples. This limited testing is not a guarantee that LBP or ACMs are limited to those areas where identified in this report. The limitations of this limited testing should be recognized as conclusions are formulated on the environmental risks associated with this property. Empirical does not warrant the work of regulatory agencies or other third parties supplying information which may have been used during the preparation of this report.

This limited LBP and ACM testing report was prepared for the exclusive use of AECOM for specific application to the subject property. No warranties, either expressed or implied, are made or intended. In the event that any changes in the nature or location of suspected LBP and ACM as outlined in this report are observed, the conclusion and recommendations contained in this report shall not be valid unless these changes are reviewed and the opinions of this report are modified or verified in writing by Empirical.

Empirical appreciates the opportunity to provide these environmental services to AECOM. If you have any questions or concerns, please call 719-749-2068.

Sincerely, Empirical Environmental, LLC

Analyst - Johann Hofer Signatory - Lab Director - Kurt Kettler Distinctly stratified, easily separable layers of samples are analyzed as subsamples of the whole and are reported separately for each discernible layer. All analyses are derived from calibrated visual estimate and measured in area percent unless otherwise noted. The report applies to the standards or procedures identified and to the sample(s) tested. The test results are not necessarily indicated or representative of the qualities of the lot from which the sample was taken or of apparently identical or similar products, nor do they represent an ongoing quality assurance program unless so noted. These reports are for the exclusive use of the addressed client and that they will not be reproduced wholly or in part for advertising or other purposes over our signature or in connection with our name without special written permission. The report shall not be reproduced except in full, without written approval by our laboratory. The samples not destroyed in testing are retained a maximum of thirty days. The laboratory measurement of uncertainty for the test method is approximately less than 1 by area percent.

Accredited by the National Institute of Standards and Technology, Voluntary Laboratory Accreditation Program for selected test method for asbestos. The accreditation or any reports generated by this laboratory in no way constitutes or implies product certification, approval, or endorsement by the National Institute of Standards and Technology. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the Federal Government. Polarized Light Microscopy may not be consistently reliable in detecting asbestos in floor coverings and similar non-friable organically bound materials.
