The following is a sample of the information contained within the past performance information form. The format set forth in this sample is not an exact duplicate of the online version (PPI Tool). As specified in Section L this PPI form template is required for each project/contract submitted as part of your proposal in addition to electronic and hardcopy of PPI tool entry.

13. Rationale for effort relevance: Describe your rationale supporting your assertion of relevance of the submitted contract to the WE&C acquisition. Clearly link the past performance information to the WE&C Basic Contract Statement of Work (SOW) and relevancy definitions. Include details that indicate performance in remote/austere/hostile environments, if applicable. Include any issues relating to emergency response, UXO/HAZMAT/HAZWASTE, types of construction and/or property control.
