PRESENT/PAST PERFORMANCE QUESTIONAIRE FORM (PPQ)
The following is a sample of the information contained within the past performance questionnaire.

The Air Force Civil Engineering Center and the Air Force Installation Contracting Agency are in the process of selecting contractors for award under the Worldwide Engineering and Construction contract. In general terms, this contract will be a focal point for acquiring a wide range of design and construction operations and services for federal customers worldwide. The length of this contract will be a minimum of 5 years. The Government wants to be confident that the contractors selected will be able to successfully accomplish our requirements.
One of the ways the Government intends to evaluate each Offeror is through the verification of the Offeror's past and present performance on other contracts. For this, we must depend on information received from agencies such as yours, which have had first-hand experience with an Offeror.
We have tried to make our questionnaire as quick and simple to complete as possible. We ask that you complete the attached questionnaire and return it directly to the Government no later than 15 calendar days prior to the date set for receipt of proposals. General contract information was provided by the Offeror (Part I). Please confirm this information and, in the space provided, identify any errors or changes you feel should be made. Then, answer the questions that follow in Part II.
We would greatly appreciate your comments describing why you rated a particular question the way you did. For any question that you rate as above or below satisfactory, we request that you provide rationale/comments to support the basis of the assigned rating.
When finished, simply email a copy of the attached to afcec.we_c.contract@us.af.mil. If you have any questions or difficulty completing the questionnaire, please contact Ms. Beatrice Torres at the same email identified above.
Please do not return or e-mail the questionnaires back to the offeror or contractor. Completed questionnaires are source selection information and must be protected. They will not be released to the offeror.

12. Please provide a brief description of service provided under this contract. Clearly demonstrate management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified. This may include a discussion of efforts accomplished by the Offeror, or applicable Teaming Partner, to resolve problems encountered on prior contracts as well as past efforts to identify and manage program risk. For example, submit quality performance indicators or other management indicators that clearly support that an Offeror, or applicable Teaming Partner, has overcome past problems. Include details that indicate performance in remote/austere/hostile environments, if applicable. Include any issues relating to emergency response, UXO/HAZMAT/HAZWASTE, types of construction and/or property control.
13. Rationale for effort relevance: Describe your rationale supporting your assertion of relevance of the submitted contract to the WE&C acquisition. Clearly link the past performance information to the WE&C Basic Contract Statement of Work (SOW) and relevancy definitions. Include details that indicate performance in remote/austere/hostile environments, if applicable. Include any issues relating to emergency response, UXO/HAZMAT/HAZWASTE, types of construction and/or property control.

Part II – To Be Completed by Reference
Please provide your opinion on the contractor’s performance for the following statements by putting a check mark in the appropriate box using the following definitions:
Outstanding – Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective. Note: To justify an outstanding rating, you should include a narrative that identifies multiple SIGNIFICANT events that illustrate the rating and state how it was a benefit to the CUSTOMER. A singular benefit could be of such magnitude, that it alone constitutes an outstanding rating. Also, there should have been no SIGNIFICANT weaknesses identified. A description of the significant events should be included in remarks.
Above Average – Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective. Note: To justify an Above Average rating, you should include a narrative in remarks that identifies a significant event that illustrates the rating(s) selected in and state how it was a benefit to the CUSTOMER Also there should have been no SIGNIFICANT weaknesses identified. A description of the significant events should be included in remarks.
Satisfactory – Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory. Note: To justify a Satisfactory rating, there should have been only minor problems, or major problems the contractor recovered from without impact to the contract. Also there should have been no SIGNIFICANT weaknesses identified. Per DoD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
Marginal – Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented. Note: To justify Marginal performance, you should include a narrative in remarks that identifies a SIGNIFICANT event that illustrates issues that the contractor had trouble overcoming and state how it impacted the CUSTOMER. A Marginal rating should be supported by referencing the management tool that notified the contractor of the contractual deficiency (e.g. Notice of Noncompliance, Letter of Concern, Performance Deficiency Letter or Cure Notice).
Unsatisfactory – Performance does not meet significant contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective. Note: To justify an Unsatisfactory rating, you should include a narrative in REMARKS that identifies multiple SIGNIFICANT events that illustrate issues that the contractor had trouble overcoming and state how it impacted the CUSTOMER. However, a singular problem could be of such serious magnitude that it alone constitutes an Unsatisfactory rating. An Unsatisfactory rating should be supported by referencing the management tools used to notify the contractor of the contractual deficiencies (e.g. Notice of Noncompliance, Letter of Concern, Performance Deficiency Letter or Cure Notice).
NOT APPLICABLE - Unable to provide a score. Performance in this area not applicable to the effort assessed.
NOTE: In order for the Government to have adequate information to support an assigned rating of above or below satisfactory, we request that you provide rationale/comments in remarks section as applicable to the below questions.
